Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number: 36C25818Q9528 Solicitation Type: Request for Quotation (RFQ) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99 6-15-2018.. This acquisition is 100% set-aside for Service Disabled Veteran-Owned Small Business Concerns that have been verified by the U.S. Department of Veterans Affairs Center for Veterans Enterprise (CVE) in accordance with 38 CFR § 74 VA Service Disabled Veteran-Owned Small Business Verification Guidelines and appear in the Vendor Information Pages (VIP) database found at http://www.vip.vetbiz.gov/. The North American Industry Classification System (NAICS) Code associated with this procurement is 333314 and the small business size standard is 500 employees. The items being requested are Zeiss Retinal Workplace 5000. All items are being requested as Brand Name or Equal. Please refer to section A.1 and A.2 for complete line item information and delivery/schedule. All items shall be delivered within 30 Days After Receipt of Order (ARO) to the following location: Southern Arizona VA Health Care System Attention: Receiving Warehouse 3601 S. 6th Ave Tucson, AZ 85723 Please mark all shipments to the attention of Philip Lampert. The provision at FAR 52.212-1, Instructions to Offerors Commercial Items applies to this acquisition. Addenda are attached to the provision. The provision at FAR 52.212-2, Evaluation Commercial Items does not apply to this acquisition. Each line item will be evaluated separately based on price. Each line item will be awarded to the lowest priced responsive and responsible offeror on an All or None basis, meaning that the offeror must be able to supply the total quantity requested of the line item to be considered for award. All offerors are advised to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications Commercial Items with their quote. The clause at FAR 52.212-4, Contract Terms and Conditions Commercial Items applies to this acquisition. Addenda are attached to the clause. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial items applies to this acquisition. Quotes shall be submitted via email to tina.kennedy@va.gov. All quotes and additional required documentation are to be received no later than 5:00 PM Eastern Time on July 9, 2018. Quotes shall consist of returning the CLIN item listing filled out with pricing that is included in Section A.1 and A.2of this solicitation.
