- CLASSIFCATION CODE: Q Medical Services; (Q201 Medical General Health Care) NAICS CODE: 621498 All Other Outpatient Care Centers Network-9 Contracting Office on the behalf of the Memphis VA Medical Center has developed this Request for Information (RFI)/Sources Sought Notice to solicit market research information from the healthcare industry to assist the Government with analysis of a Government requirement and provide commercial insight on best practices and potential issues. THIS IS A REQUEST FOR INFORMATION ONLY. It is not a Request for Proposal, a Request for Quotation, an Invitation for Bids, a Solicitation, or an indication that Network-9 Contracting Office (NCO-9) will contract for the items contained in the RFI / Sources Sought Notice. This RFI/Sources Sought Notice is part of a Government market research effort to determine the scope of industry capabilities and interest, in accordance with Federal Acquisition Regulation (FAR) Part 15.201(e). Responses to this RFI / Sources Sought Notice are not offers and cannot be accepted by the Government to form a binding contract. Responses to this RFI / Sources Sought Notice are strictly voluntary and are consider as information only. The Government will not pay respondents for information provided in response to this RFI / Sources Sought Notice. Responses to this RFI / Sources Sought Notice will not be returned and respondents will not be notified of the result of the review. If a solicitation is issued, it will be announced on the Federal Business Opportunities Website http://www.fbo.gov at a later date, and all interested parties must respond to that solicitation announcement separately from any response to this announcement. This RFI / Sources Sought Notice does not restrict the Government s acquisition on a future solicitation. Network-9 Contracting Office (NCO-9) and Memphis VA Medical Center is conducting market research for a Contractor(s) to provide Community Based Outreach Clinic (CBOC) to provide Primary Health Care (PHC) and Mental Health Care (MHC) for approximately 4,500 Veteran beneficiaries of the Department of Veterans Affairs (VA) and the Memphis VA Medical Center, 1030 Jefferson Avenue, Memphis, TN 38104-2127. GENERAL: Services Required The Memphis (TN) Veterans Affairs Medical Center (Memphis VAMC) the parent facility requires CBOC services including the provision of Primary Health (PH) and Mental Health (MH) services in a private hospital, office or clinic environment to Veterans, primarily residing in the following Tennessee/MS counties: DeSoto, Benton, Lafayette, and Marshall Counties. The current contract for services is expiring and the Memphis VAMC has identified a need to continue receiving these services for Veterans residing within the county coverage area for this CBOC. The CBOC is established to improve access to veterans in rural areas to primary and specialty care. The CBOC will be located in an area that will greatly reduce the veteran's drive time. The CBOCs highest priority is to improve access to care for high priority enrolled veterans. Improved quality of care will reduce waiting times for follow-up care, in addition to improve waiting times at the parent hospital. The CBOC will be serving patients in a medically underserved manpower and a Primary Care Health Professional Shortage Areas. The Contractor shall provide a primary CBOC facility in or around the city of Holly Springs, MS and within 5- to 10-miles of a local hospital providing emergency room services. Description of Services to be provided: Scope of Services The medical care provided will consist of those services commonly associated with a general medical office visit. A staff physician, registered nurses, and/or others will provide primary care and preventive medicine services consisting of intake, initial assessment, disease prevention and treatment, overall care management, and patient/caregiver education for veterans. Primary care provides for the diagnosis and treatment of diseases along the full spectrum of illness, including acute management to chronic care with an increased focus on primary prevention and, once illness has occurred, prevention of complications. Preventive services may include screening, counseling, immunizations, and educational interventions. The primary care provider refers patients to specialty clinics for evaluation and/or treatment as appropriate. Referrals are also made for rehabilitation, home care, alternative levels of care, and other needs as appropriate. Blood draws will be accomplished at the clinic and sent to VAMC Memphis for processing in the medical center lab. The primary care provider will support managed care and will be required to refer patients to the Memphis VAMC (parent facility) for any specialty consultations, diagnostic testing beyond the scope of the clinic, or hospitalizations. Ancillary services such as routine laboratory procedures will be provided by the parent facility (blood draws will be accomplished at the clinic). Medications will be provided by the parent facility. The primary care providers will use "group clinics" to optimize time and manage chronic disease. The ACOS/Ambulatory Care at the Memphis VAMC is clinically responsible for primary care services. A clinic physician manager will have primary administrative responsibility for the clinic s day-to-day operations. Primary care providers cooperate with other services located at the VA Medical Center to evaluate and successfully treat patients. All caregivers and trainees will integrate training and research activities with direct patient care and will utilize technological advances to provide coordinated comprehensive care. Emergency medical services will not be available at this clinic; however, an Automatic External Defibrillator (AED) will be available at the clinic. In a medical emergency situation, clinic staff will contact local emergency management services. After hours, patients are instructed to contact the VISN 9, After Hours Call Center. Mental Health Service Assessment Veterans needing basic mental health services including screening and prevention for mental disorders, and diagnostic evaluation for mental illness and substance abuse; pharmacotherapy, psychotherapy and/or psychosocial counseling for mental disorders; and referral for inpatient or residential care, direct care, or access to consultation for special emphasis and/or complex problem (e.g., Post-traumatic Stress Disorder (PTSD), substance abuse treatment, sexual trauma counseling, and patient and/or family education) management of psychiatrically prescribed medications and referral of complex cases will be provided on site and more intensive mental health services referred to the Memphis VAMC. The plan is for mental health services to be available for approximately 30% of all users. The initial plan calls for a.5 mental health provider and a.5 Social Worker based on approximately 1,200 enrolled veterans. The option of tele health will be implemented as soon as the clinic opens. Support for tele health initiatives would come from VAMC Memphis mental health staff. Specialty Care Needs Assessment Specialty services where there are limited area resources such as Dermatology and Rheumatology may be addressed through the incorporation of tele health initially between the CBOC and Memphis VAMC. Initial equipment and communication estimates include cost for T1 lines and tele health equipment. Access to special disability services, such as SCI, Blind and low vision services, prosthetics will be supported by the parent facility. Support Service and Relationship The parent facility is the Memphis VAMC, located approximately 30 miles away in Memphis, Tennessee. Support for specialty services, administrative operations and general clinic operation will be under the ACOS for Ambulatory Care Services who reports directly to the Chief of Staff. Staffing support during unexpected absences will come from the parent facility. After hours emergency care will be provided as per VA and medical center policy in the local community. Quality of Care Monitoring Quality of care in the clinic is monitored on a continuing basis. This clinic is included in the External Peer Review Program that supports VHA s Performance Measurement Program. The Office of Quality Management and Performance Improvement will provide feedback in CBOC performance with regards to preventive care, chronic disease management, and clinical guidelines. Quality measures include cancer, cardiovascular, endocrinology, diabetes, infectious diseases, and mental health. CBOC patients are included in national (SHEP) and local patient satisfaction surveys.
