Description
THIS IS A SERVICE DISABLED AND VETERAN OWNED SMALL BUSINESS SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS   The Department of Veterans Affairs VISN 7 Network Contracting Activity is conducting a MARKET SURVEY to obtain information regarding the availability and capability of qualified Veteran Owned Small Business sources whether they are: (1) Service-Disabled Veteran-Owned (SDVOSB), (2) Veteran-Owned Small Business (VOSB), and their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. Small business concerns should be capable of delivering the various optometry equipment requested by the Charlie Norwood VA Medical Center (VAMC), Augusta, GA. The NAICS: 811310 DESCRIPTION OF THE REQUIREMENT: The intent of this requirement will be to establish a contract to furnish F.O.B. destination 1abor, material, parts, equipment, manuals and schematic drawings necessary to perform preventive maintenance service, emergency repair work and performance evaluations for the following equipment at the specified locations: LOCATION / EQUIPMENT TO BE MAINTAINED: 1. Charlie Norwood, UPTOWN DIVISION, 1 Freedom Way, Augusta, GA 30904 Emergency Generator. Genset #1, Mitsubishi - Model SI2A2-PTA, SN20084, Building 110 Emergency Generator, Genset #2, Mitsubishi - Model SI2A2-PTA, SN20084, Building 110 Emergency Generator, Genset, Cummins - Model NTA-855-GS.SN 10836207, Building 81 Portable Generator 40kw, Building 84 2. Charlie Norwood, DOWNTOWN DIVISION, 950 15th St., Augusta, GA 30901 Emergency Generator, EG- 1, Caterpillar - Model #D399, SN 3583846, Building 802 Emergency Generator, EG-2, Caterpillar - Model # D399, SN 35B3845, Building 802 Life Support Generator, LSG, Caterpillar - Model #3412, SN 38S3234, Building 801 Fire Pump Model #N855F, SN 10795179, Building 802 MAINTENANCE SCHEDULE AND REQUIRED TASK: 1. Preventive maintenance schedule (PMI) shall be on semi-annual basis. PMI visits must not be less than 180days apart. 2. Each maintenance or emergency repair event will include inspection for internal and/or external causes of medical and electronic or component problems) including: a. Wear, misalignment, maladjustment, damage or other malfunctions with respect to the manufacturer specifications. b. Inspection for electrical safety and fire hazards. c. Cleaning and lubrication of equipment as required. d. Repair or replacement of all defective components necessary to conform to instrument specifications. e. Calibration or standardization of instrument required. f. Testing for proper operation of entire system and subsections. g. Document proper function and performance of instrument. 3. The equipment shall be maintained in optimum operating condition and meet manufacturer's specification for operation and performance. All work shall be performed in accordance with the terms and conditions contained herein. Contract period shall be for one (1) year from the effective date of award with four option periods. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. 2) Provide a Statement of Capability that demonstrates the offeror's past performance in delivering like products and equipment. Please include the following: (a) prior completed projects of similar nature; (b) corporate experience and management capability; and (c) examples of prior completed Government contracts, references, and other related information. 3) Submit Capability Statements NLT Tuesday, July 10, 2018 @ 12:00 pm. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities (FBO). However, responses to this notice will not be considered adequate responses to a solicitation.
