Description
Central Arkansas Veterans Health Care System Eye Lane Equipment for Hot Springs VA Eye Clinic Scope The will be received at CAVHS warehouse, Bldg 182, at 2200 Fort Roots Drive, North Little Rock, Arkansas. After inspection and internal identification by Biomed Department it will be placed in the satellite CBOC Eye Clinic upon completion of the expansion construction at 177 Sawtooth Oak St, Hot Springs, AR  71901. The scheduled date of completion of construction will be announced upon award. The eye lane equipment will provide standardization of eye exams. With the addition of this new clinic, patients can be seen locally. This will allow them to be seen in a more timely manner and take some of the load off the Little Rock Eye Clinic. Also, this equipment will allow patients to be seen because this equipment allows providers and technicians to perform routine eye exams, diagnosis and treat diseases of the eye. NO INSTALLATION INCLUDED THE REQUESTOR NEEDS BP cuffs to have in case of emergency for pt. care THE REQUESTOR NEEDS two head pieces and two handles-one for each room. The handpieces are rechargeable and occ fail. The heads are removable and can be placed on different handles and having more handles helps us not have to remove the head from use to use and Thus cutting down on lost or broken heads. Eye lanes are a stand-alone system. Specifications The Eye Exam equipment conforms to all applicable safety standards such as IEC 60601-1, IEC 60601-1-2, IEC 60601-1-4, UL 60601, and IEC 60825. The basic components of the Eye Lane are: Exam chair Foot switch for overhead light, plugs directly into wall Stand for slit lamp, also plugs directly into wall. Projector that plugs directly into wall Power Supply Model PWS / FW, IEEE 1394 cable to PC Operation Manuals Hardware Operation Instructions Manual Installation and System Configuration None of the eye lane equipment runs by computer. Maintenance The non-optical surfaces of the equipment can be cleaned and/or disinfected whenever necessary. Any standard cleaning product appropriate for plastic surfaces that does not contain acetone or hydrogen peroxide (e.g. ethanol and isopropyl alcohol disinfectants) may be used for this purpose. Training No training required as this is standard equipment in all eye lanes. Delivery & Installation All items will be delivered per VA Policy. Installation to be coordinated by (to be announced upon award) Security Privacy Officer, Info Security Officer and Records Manager s statements: Information Systems Officer, Information Protection:   The contractor will/will not have access to VA Desktop computers and they will/nor will they have access to online resources belonging to the government while conducting services. If removal of equipment from the VA is required, any memory storage devices, such as hard drives, solid state drives and non-volatile memory units will remain in VA control and will not be removed from VA custody.     Privacy Officer:   The contractor will/will not have access to protected Patient Health Information (PHI) and they will/nor will they have the capability of accessing patient information during the services provided to the VA and if removal of equipment from the VA is required, any memory storage devices, such as hard drives, solid state drives and non-volatile memory units will remain in VA control and will not be removed from VA custody.     All research data available for Contractor analyses is de-identified. Records Manager:     1.     Records Manager:   a.     Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. b.     Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. c.     Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. d.     Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. e.     Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. f.       The Government Agency owns the rights to all data/records produced as part of this contract. g.     The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data.
