Solicitation Number: 36C25818Q9544 Solicitation Type: Request for Quotation (RFQ) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation 36C25818Q9544 is issues as an invitation to request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 06-15-2018. This acquisition is 100% set-aside for Service Disabled Veteran Owned Small Business (SDVOSB) Concerns that have been verified by the U.S. Department of Veterans Affairs Center for Veterans Enterprise (CVE) in accordance with 38 CFR § 74 VA Veteran-Owned Small Business Verification Guidelines and appear in the Vendor Information Pages (VIP) database found at http://www.vip.vetbiz.gov/. The North American Industry Classification System (NAICS) Code associated with this procurement is 333241 Food Product Machinery Manufacturing and the small business size standard is 500 employees. The Contract Line Item Numbers (CLINS), items, quantities and units of measure requested are: CLIN ITEM QUANTITIY UNIT OF MEASURE 0001 Air Curtain Refrigerator PHB12DS 4 EA The items being requested are to replace the four end of life cycle cabinet refrigerators, Carter-Hoffmann PHB12DS. Item being requested is Brand Name or Equal. The Salient Characteristics of the item are as follows: The cabinet refrigerator cabinets shall have the following technical specifications: Keep food cold at maximum allowable 90 minutes with door open Operator warning and automatic shutdown Clean condenser reminder light and on demand defrost with defrost cycle indicator light Defrost element shall be 750 watts An adjustable temperature ranges from 33 degrees to 38 degrees F. Sliding space saver door, tucks compactly into side of unit minimizing door swing and saving space having to adjust the cabinet to accommodate door swing. Indicator lights for temperature and cleaning of condenser. Stainless steel 20-gauge polished exterior 22-gauge stainless steel interior with easy to clean coved corners. Handles form grip flush in wall hand grips recessed mounted on each side Rear mounted tubular Tray slides must secure tray and support with tilt mount feature for removable cleaning MUST hold up to 10 full size trays 18x26 or 20 half size trays 14x18 width a 4.5 standard spacing with tilt feature Height of cabinet 78.75 and 37 deep and 32 wide 3 wire grounded power cord and plug, rear mounted for safety, Volts 120 Watts 1080, amps 9, Hertz 60 and Phase single. Plug 5-15P. The air curtains shall be new, not used, and not refurbished All items shall be shipped Free on Board (FOB) Destination and delivered on or before Aug 15. 2018 at the below location: Northern Arizona VA Health Care System Attention: Warehouse 500 Hwy 89N Prescott AZ 86313-5001 Please mark all shipments to the attention of Keca Chapman, NAVAHCS, Nutrition and Food Service Department. The provision at FAR 52.212-1, Instructions to Offerors Commercial Items applies to this acquisition. Addenda are attached to the provision. The provision at FAR 52.212-2, Evaluation Commercial Items does not apply to this acquisition. This acquisition will be evaluated based on price. This acquisition will be awarded to the lowest priced responsive and responsible offeror on an All or None basis, meaning that the offeror must be able to supply the total quantity requested of the acquisition to be considered for award. All offerors are advised to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications Commercial Items with their quote. The clause at FAR 52.212-4, Contract Terms and Conditions Commercial Items applies to this acquisition. Addenda are attached to the clause.
