2 2 Northport VA Medical Center, Northport NY (632) Operating Room The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 2), James J. Peters VA Medical Center, Bronx New York, intends to award a contract for Medtronic, Valley Lab FT10 Generator to the Veterans Affairs, Northport VAMC, 79 Middleville Road, Northport, NY 11768. This action will result in a Firm-Fixed Price contract. This source sought notice is not a request for quotations; interested parties may express their interest by providing a capabilities statement NLT Friday July 13, 2018 at 2:00 PM EST. The capabilities statement must provide clear and unambiguous evidence to substantiate the capability of the party to provide the required supplies and services. Verbal responses are not acceptable and will not be considered. When responding to this announcement, respondents should refer to Announcement 36C242-18-Q-9598_ _ Medtronic, Valley Lab FT10 Generator Sources Sought Notice. If after July 13, 2018, no ACCEPTABLE responses have been received in response to this announcement, the Department of Veterans Affairs shall negotiate solely with Medtronic. The NAICS is 334510 Electromedical and Electrotherapeutic Apparatus Manufacturing. NAICS 334510 is on the SBA Class Waiver List. However, NAICS 334510 is not in compliance with the Non Manufacturer Rule (NMR) because this is a brand name only request. This sources sought notice is not a request for competitive quotes. Upon collection of all written responses received, there will be a determination made as to the most appropriate solicitation approach for this Northport VAMC requirement. The Government anticipates the award of a firm-fixed- price contract. A determination by the Government whether to compete this proposed requirement will be based upon responses to this sources sought notice and will be solely within the discretion of the government. All written responses and information received will normally be considered solely for the purpose of determining whether to conduct a competition for this Northport VAMC requirement. STATEMENT OF WORK (SOW) Valley Lab FT10 Generator 1. Introduction and Overview The Northport VAMC has five operating rooms and is an extremely busy department requiring replacement and additional capital equipment purchases. 1.1 Background Currently, there is an increased workload of cases and as such Northport VAMC requires the purchase of additional medical equipment for complex cases. 1.2 Scope of Work Acquire one Medtronic, Valley Lab FT10 Generator. Objectives The primary objective is to purchase one Valley Lab FT10 Generator. The equipment provides RF energy for monopolar and bipolar surgical applications, and tissue-fusion and vessel-sealing applications. It features a touchscreen divided into four sections for viewing and user input of settings and options available for any application. The energy platform automatically detects coded handsets and configures the energy platform accordingly. Period of Performance- Delivery to Northport VAMC 2. References- N/A 3. Requirements Equipment to be delivered upon receipt to medical center. Statement of warranty of equipment, minimum one year. 3.1 Tasks Deliver one Valley Lab FT10 Generator to the Northport VA Medical Center, Building 36 Warehouse, 79 Middleville Road, Northport, NY. 11768 3.2 End Results/Deliverables- N/A 3.2.1 List of Deliverables by Task- N/A 3.3 Schedules/Milestones Delivery of one tissue processor within 30 days of purchase. Validation of performance within 30 days of delivery. Other Considerations- N/A 4. Progress/Compliance- N/A 5. Transmittal/Delivery/Accessibility The contractor shall provide [1] hard copy of each deliverable and one electronic version as well as user s manual.
