Page 1 of 1 Page 2 of 2 Combined  Synopsis/Solicitation  Notice The purpose of this Combined Synopsis/Solicitation Notice is to notify interested sources that the Department of Veterans Affairs (VA Mid-Atlantic Health Care Network, Veterans Integrated Service Network (VISN) 6) is seeking proposals from interested sources to provide all labor, material, tools, equipment, maintenance and repair of all Contractor furnished equipment (including a mobile trailer/unit), specialized staff, training, required supplies, and medications/drugs necessary to perform Positron Emission Tomography/Computerized Tomography (PET/CT) Services at Hampton, VA (VAMC) and Salem, VA (VAMC) with options to service additional medical centers/outpatient clinics within VISN 6 as needed. This requirement is a total Service Disabled Veteran-Owned Small Business (SDVOSB) Set-Aside. The performance period will be from April 1, 2019 through March 31, 2020 with four (1) year option periods. The North American Classification System (NAICS) Code 621512 (Diagnostic Imaging Centers) with a size standard of $15.0M or less applies to this requirement. Contractor furnished equipment shall include but not be limited to a fully equipped PET/CT Mobile Trailer where studies will be performed on a PET/CT scanner manufactured in or after 2007 and a PET/CT review station and fusion software to be placed within each medical center obtaining service. The types of exams expected to be performed on the Contractor provided PET/CT system shall include but not be limited to: oncology related PET/CT exams, imaging of cardiac metabolism with F-18 FDG, and brain imaging with F-18 FDG. The Offeror shall meet all Federal, State and Local codes and requirements for the operation of this service and equipment provided. Offerors personnel performing services shall be licensed and/or registered in a State, Territory or Commonwealth of the United States or the District of Columbia and meet Nuclear Medicine Technologist professional qualifications. Offerors personnel shall include, but not be limited to certified Nuclear Medicine Technologists (ARRT and/or NMTCB) and support and driver staff. Offerors who are interested and capable of providing the above services must demonstrate their experience and knowledge of the required services and their capability to provide such services within their proposal. Offers shall follow the Instructions for Proposal Submission located within the attached RFP 36C24618R0818 and include all required documents. Offerors should submit all technical questions regarding this solicitation to the Contracting Officer in writing on or before July 16, 2018 by 9:00am (EST). No question will be accepted after this date. Questions must be sent via e-mail to larry.lapp@va.gov.
