- Notice Type: Posted Date: Sources Sought July 2, 2018 Original Set Aside: Response Date: Tier Evaluation July 9, 2018 @ 1:00 p.m. EST Priority SDVOSB through Small Business Classification Code: NAICS Code: V224 Ambulance Transportation 621910 Ambulance Transportation $15 Million Size Standard Sources Sought Synopsis: THIS IS A SOURCES SOUGHT ANNOUNCEMENT to conduct market research for Non-Emergent Ambulance Transportation Services for local and out of town transportation twenty-four (24) hours a day, seven (7) days a week on behalf of the WJB Dorn VA Medical Center and seven (7) Community Based Outpatient Clinics (CBOCs) located in Columbia, South Carolina and its surrounding area. The facility intends to award a firm-fixed priced IDIQ service contract for a Base Year to include the possibility of four (4) Option Year periods to begin November 1, 2018. The purpose of this sources sought is to locate all possible sources who are technically capable, hold experience in Ambulance Transportation service, and would like to be considered if a solicitation is advertised for this particular type of service. Highway 81 Columbia, SC 29209 Anderson, SC 29621 Sumter CBOC Florence CBOC 407 North Salem Avenue 1822 Sally Hill Farms Blvd Sumter, SC 29150 Florence, SC 29501 Greenville CBOC Rock Hill CBOC 41 Park Creek Drive 2670 Mills Park Drive Greenville, SC 29605 Rock Hill, SC 29732 Orangeburg CBOC Spartanburg CBOC 1767 Village Park Drive 279 North Grove Medical Park Drive Orangeburg, SC 29118 Spartanburg, SC 29303 BRIEF CAPTION PERFORMANCE WORK STATEMENT Non-Emergent Ambulance Transportation Services Contractor shall provide Ambulance Transportation Service, to the maximum extent possible, by honoring Transportation Service Requests made twenty-four (24) hours in advance. Contractor shall be able to accept request for same day service Contractor shall be able to schedule requested service in a manner that meets the arrival time specified as required for the patient s appointments. There shall be only one (1) patient transported during each trip unless specifically authorized by the Special Mode Transportation (SMT) Coordinator, COR, or Medical Administrative Officer on Duty (AOD). The Contractor is required to pick up the patient as near to the scheduled pickup time as possible. If Contractor arrives earlier than the scheduled pick-up the Driver shall wait at the pickup location until five (5) minutes past the scheduled pickup time before calling the Dispatcher for instructions. Contractor shall provide same day Transportation which shall be available at the destination within thirty (30) minutes from the time the transport is arranged. If the Contractor fails to furnish Ambulance Service within thirty (30) minutes after receiving a request of any order, the Government reserves the right to obtain the service from another source and to charge the Contractor with any excess cost which may result thereof. In no instances shall the Contractor be required to furnish more than twelve (12) Ambulances for one-time period at any of the facilities. All Drivers are required to report to the Special Mode Transportation (SMT) Coordinator, COR, or Medical Administrative Officer on Duty (AOD) at any time a pick-up, delivery, or when departure is made. Each Ambulance shall have Patient Compartment Facilities, Oxygen and Suction Systems and Environmental Climatic Supplies as required by Federal Specification for Ambulances, KKK-A-1822D, KKK-A-1822C and KKK-A-1822E. All Ambulances shall be five (5) years or newer and shall meet the standards of KKK-A-1822D. All other Ambulances shall meet the standards of KKK-A-1822C. Each Ambulance utilized to provide services under this contract shall be a Type I, Type II or Type III Ambulance per Federal Motor Vehicle Safety Standard No.108 and Federal Ambulance Specification KKK-A-1822. The Government reserves the right to thoroughly Inspect all Ambulance Vehicles that shall be providing transportation services. All Ambulance Vehicles that are to be utilized for this service shall be inspected within two (2) weeks prior to commencement of this service by the delegated COR. Certification of Insurance required Ambulance and Personal Liability Contractor is required to provide the following personnel and requirements: EMTs Certification/License for South Carolina Minimum of 127 Hours of Training Paramedics Certification/License for South Carolina Minimum of 424 Hours of Training Driver s current License in the state of South Carolina Not to exceed five (5) current points none for Reckless Driving This procurement is set-aside based on an order or priority as established in 38 U.S.C. 8127. This solicitation is being issued as a tiered evaluation for SDVOSB concerns, or in the alternative, a set-aside for VOSB concerns, or in the alternative, a set-aside for other small business concerns with HUBZone small business concerns and 8(a) participants having priority. If award cannot be made, the solicitation will be cancelled and the requirement resolicited. The tiered evaluation will be conducted as follows: Tier 1: SDVOSB concerns Offers, if any will be evaluated. If no offers were submitted by SDVOSB concerns, or if none of the offers would result in award at a fair and reasonable price that offers best value to the United States, the government will then move to evaluation of Tier 2 offers. Tier 2: VOSB concerns Offers, if any will be evaluated. If no offers were submitted by VOSB concerns, or if none of the offers would result in award at a fair and reasonable price that offers best value to the United States, the government will then move to evaluation of Tier 3 offers. Tier 3: HUBZone & 8(a) Small Business concerns Offers, if any will be evaluated. If no offers were submitted by HUBZone & 8(a) concerns, or if none of the offers would result in award at a fair and reasonable price that offers best value to the United States, the government will then move to evaluation of Tier 4 offers. Tier 4: All other Small Business concerns - Offers, if any will be evaluated. If no offers were submitted by any Small Business concerns, or if none of the offers would result in award at a fair and reasonable price that offers best value to the United States, then the solicitation will be cancelled and the requirement resolicited.
