- REQUEST FOR INFORMATION / SOURCES SOUGHT INQUIRY The Department of Veterans Affairs Network Contracting Office (NCO) 19, Rocky Mountain Acquisition Center (RMAC) is seeking information and sources that can provide on-site Intraoperative Neurophysiologic Monitoring (IONM) services for its Veteran population at VA Salt Lake City Health Care System (VASLCHCS). Place of Performance: The Contractor shall perform IONM services at the VASLCHCS, 500 Foothill Drive, Salt Lake City, UT 84148. Qualifications: Contractor shall provide qualified technicians for IONM services in an operating room (OR) setting for the SLCVAHCS. IONM services shall include all hardware, software, peripherals, instructions and manuals required to support contractor monitoring equipment. Contractor shall also furnish all preventive maintenance, routine servicing and quality control necessary to ensure optimum operation of its equipment. Contractor should possess staffing sufficient to support concurrent on-site surgeries at SLCVAHCS while ensuring availability of additional IONM services within two (2) hours of notification in emergency situations. IONM technicians and neurologists shall possess experience and proficiency in multiple specialties such as spinal, otylaryngeal (ENT), cranial, vascular and peripheral nerve. ALL proposed personnel for shall be approved in advance, having met the qualification AND substantiating documentation requirements. SLCVAHCS anticipates a need of services for 120 cases per year. Contractor shall provide IONM services for up to three (3) concurrent cases during normal business hours. a. Contractor shall have written contingency plans in place to cover emergencies, unexpected events, and assigned KEY PERSONNEL being located outside a 2-hour driving perimeter of Salt Lake City VA Medical Center. b. Contractor shall provide direct line phone numbers bypassing menus, recorded messages, etc., providing authorized Government personnel immediate access to contractor scheduling and/or customer service personnel 24/7/365. Voice/live response to customer calls shall not exceed thirty (30) minutes. Type of Contract: If this request for information leads to a future solicitation, the Government intends to award a firm-fixed price contract. The performance period shall be one (1) base year with four (4); one (1) option years. This is a request for information and sources only, which may or may not lead to a future solicitation! This is not a request for proposal (RFP). The VA will not pay for any information received resulting from this sources sought notice. Requests for copies of a solicitation shall not be honored or acknowledged. Information should be forwarded to the Contract Specialist.
