- Statement of Work Proposed Q & R Welcome Brochure Plan Benefits & Coverage Solicitation Package (SF-1449 Form and Provisions & Clauses) Open Enrollment Instructions Overivew The U.S. Merchant Marine Academy (USMMA) seeks a qualified insruance company (Carrier) to provide Student Health Insurance Policy (SHIP) to the general population of USMMA students, referred to as Midshipmen. The health insurance coverage would be provided while on or off campus 24 hours a day, 7 days a week, worldwide. The USMMA is interested in utilizing a Preferred Provider Organization (PPO) design. Under a hard waiver program, USMMA requires a full time undergraduate Midshipmen to carry health insurance that meets specific requirements. Plans must meet current and future State and Federal Requirements including provisions of the Affordable Care Act. Locaility Requirement Minimal coverage is required in the New York Long Island area. The USMMA also requires that in addition to meeting minimal coverage needs in New York/Long Island that coverage includes providers who practice within 10 Miles of the USMMA within the Nassau County area. This includes emergency & non-emergency coverage. Perod of Performance The Insurance contract will be issued for a base period from July 2018 to December 2019. Three (3) option years will be available upon concurrent agreement between the contractor and the Government. Each subsequent coverage period will be from January 1 to December 31. Policy & Premiums Midshipmen that enroll in the contractor provided SHIP and pay the premium, a bona fide insurance contract will exist between the Midshipman and the insurance company. The Midshipmen will be responsible for the payments of the policy premiums directly to the Carrier. USMMA will not be a party to this reference contract between the Midshipman and the insurance company. The Carrier will be responsible for obtaining and tracking all payments made by the Midshipmen. USMMA will provide assistance to the contractor regarding locating and communicating with any Midshipman. Price Adjustments: The USMMA seeks realistic initial year premium quotations with normal rate fluctations in subsequent years. Rates may be revised annually at the beginning of each new contract year. Any revision in annual rates must be justified and made available for USMMA's review and approval. Evaluation Criteria Factor 1: Technical Factor 2: Price An award will be made to the responsible Offeror whose offer, conforming to the solicitation, is determined to be the best overall value, price and other factors considered.
