Description

SOW, Tissue Stainer THIS IS A REQUEST FOR INFORMATION (RFI) ONLY This is not a Request for Proposal (RFP) or Request for Quotation (RFQ) In accordance with FAR 15.201, the Indian Health Service (IHS), Phoenix Indian Medical Center (PIMC), located at 4212 N. 16th St, Phoenix, AZ 85016, is conducting a market survey to identify contractors for the following: Preventative Maintenance Services (PM) to be provided at PIMC on the following equipment (refer to attached SOW for additional information): Sakura Tissue Stainer Model: Prisma A-10 S/N: 61300240-0207 Interested contractors who meet the criteria specified in the SOW may submit information to the acquisitions department. This RFI is open from 6/5/2018 - 6/8/2018. If interested, please submit the following: 1. If your company can successfully provide and perform the services. 2. If your company is a small or large business. 3. Indicate if your company is registered in SAM (sam.gov) 4. Your company's Data Universal Number (DUNS) This RFI is for planning purposes only. This sources sought notice/RFI is not to be construed as a commitment by the Government, nor will the Government pay for any information solicited.
