Description
SF30-MOD 1 The Navajo Area Indian Health Service, Gallup Indian Medical Center (GIMC), Purchasing Department has a requirement for a Non-Personal Performance-Based type contract for Consultant Services for the Zero Suicide Initiative program, Gallup Indian Medical Center, Gallup, New Mexico. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this acquisition is 020-GZS-18-0109. The solicitation is issued as a request for quotation (RFQ) using FAR part 12 Commercial Items and FAR part 13 Simplified Acquisition Procedures. This solicitation document and its incorporate provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-97, effective January 24, 2018. This solicitation is a total Small Business set aside. The North American Industry Classification System (NAICS) Code for this acquisition is 541611 - Administration Management and General Management Consulting Services of $15.0 Million. The Contract Line Items (CLIN) and pricing structure are as shown below. CLIN 001 Pre-program Evaluation $____________ CLIN 002 In-program of Evaluation $____________ CLIN 003 Final Program Evaluation Report $____________ All services are to be performed in accordance with the attached Performance Work Statement (PWS). Proposal/Quote - Fee Schedule: The quoted unit price shall be inclusive to provide for all labor, technical knowledge, material, supplies, supervision and all other necessary items to provide services. This includes all applicable, Federal, State and Local Taxes. Place of Performance: Zero Suicide Initiative Program will be provided within Gallup Service Unit. Period of Performance: The period of performance will be for one (1) base year with two (2) one (1) year option periods. First Year Option Date of award through October 31, 2018 Second Year Option November 1, 2018 through October 31, 2019 Third Year Option November 1, 2019 through October 31, 2020 The provision at 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. The provision at 52.212-2, Evaluation-Commercial Items, applies to this RFQ, and the Government will award a contract resulting from this RFQ to the responsive and responsible offeror whose quote conforms to the RFQ and is most advantageous to the Government. In addition to price, the following non-price factors will be considered: 1. Past Performance Evaluation will be based on the extent to which the Offeror has demonstrated; through recent past performance under contracts of similar nature, its ability to successfully meet the requirements. The evaluation will consider the Offeror's company's history of relevant experience, reasonable and cooperative behavior, adherence to contract schedule, and commitment to providing quality services similar to that described in the Performance Work Statement. 2. Key Personnel a. The Contractor shall have a minimum of 24 months of experience in providing services of a similar nature supported with a resume and certification/licensures. b. The Contractor shall have the resources and authority to ensure efficient and timely program execution and shall be the focal point for all required program tasks. c. The Contractor shall be prepared at all times to present and discuss the status of program activities, requirements, and issues. d. Experience evaluating complex community health programs. 1) Specific experience with Native American Suicide prevention. 2) Knowledge and experience deploying varied approaches to evaluation, including survey's, interviews, focus groups, participant observation. 3) Ability to use a variety of analytic tools such as statistical analysis, qualitative data analysis, on-line survey's and polling software. e. Knowledge of social science and medical understanding of suicide. f. Knowledge of organization of federal health care system. g. Education and ability to interface and coordinate a broad range of health professionals, including physicians, psychologists and social workers. h. Knowledge and understanding of the Zero Suicide Initiative approach to suicide prevention (via health systems coordination). i. Knowledge and understanding of the Zero Suicide Initiative as it applies to Gallup Indian Medical Center, which includes a system-wide approach with interventions at multiple junctures in the community, screening, treatment, referral, transitions, and follow-up. j. Ability to coordinate with the national IHS Zero Suicide Initiative Task Force and coordinator. k. Ability to submit required evaluation documents in a timely fashion as required by the Grant. 3. Logistics In evaluating the offeror's capability, the Government will consider the offeror's experience with providing healthcare services to Native American community (Navajo, Zuni, Hopi, etc.). The Government will also evaluate the proximity to provide services to the community. Offerors must submit representations and certifications in accordance with the provision at FAR 52.212-3 Offeror Representations and Certifications-Commercial Item.
