- DESCRIPTION/SPECIFICATIONS/WORK STATEMENT Provide all labor, materials, equipment, tools and reports required to provide audits and equipment calibration of the Medical and Research Radiation Safety Programs at the Clement J. Zablocki VA Medical Center (VAMC), 5000 W. National Ave., Milwaukee, WI 53295. The quarters for audits are designated as January March, April June, July September and October December. SCOPE OF WORK TO BE PERFORMED: Provide quarterly audits of VAMC Radiation Safety Program as required by 10 CFR 20.1011(c) including audits of: Policies, procedures, records, As Low As Reasonably Achievable (ALARA) license, radioactive waste, Qualitative Management Program (QMP), therapeutic dose, survey and wipe test records, shipping and receiving records compliance with Department of Transportation (DOT) regulations, manuals as identified for adherence to Nuclear Regulatory Commission (NRC) License requirements. Provide annual calibration of radiation safety survey instrumentation to include pick-up, calibration, and return of instruments within a 24-hour turnaround for the following (or equivalent) instrument inventory: Two (2) - Ion chambers Twenty (20) - Geiger-Mueller (GM) exposure rate survey meters Two (2) - Count rate survey meters with dual probes (hand and foot) to be calibrated onsite. One (1) - Scaler rate meter with up to 4 possible detectors Two (2) - Electronic dosimeters Provide annual accuracy test of one (1) dose calibrator located in the Nuclear Medicine Department. All testing is conducted within regulatory requirements using National Institute of Standards and Technology (NIST) traceable sources. Immediate notification to the Radiation Safety Officer (RSO) of any occurrences requiring external notification or corrective action. SERVICE Service shall consist of a minimum of quarterly inspections of the indicated sites. Frequency for testing and calibration of equipment is noted in the scope of work and will indicate that the equipment is functioning properly according to regulatory requirements and/or manufacturer s specifications. The contractor shall provide all labor, materials, travel, supervision, tools and facilities required in this contract. Contractor shall ensure that all work performed is in conformance with the latest published edition of applicable standards and/or regulations, such as: NRC, NFPA 99, UL, ANSI, OSHA, VA, IEST, ISO, CDRH, etc. The contractor shall provide interim calls as requested by the Contracting Officer, Contracting Officer s Representative (COR), or RSO. No one else is authorized to call in the contractor for service. Scheduled work required in the performance of this contact shall be performed during the hospital s regular hours of 7:00 a.m. and 4:30 p.m., excluding federal holidays. The contractor shall notify the COR at least 7days prior to the normal testing/inspection cycle visit and 8 hours prior to any emergency activities. Federal Holidays observed by the VAMC(s) are: New Years Day Labor Day Martin Luther King Day Columbus Day Presidents Day Veterans Day Memorial Day Thanksgiving Day Independence Day Christmas Day REPORTING REQUIREMENTS: The contractor will report to the RSO, COR, or designated alternate to log in each day. At the end of each day the contractor will log out with the RSO, COR, or designated alternate. This check in/check out procedure is mandatory. REPORTING REQUIRED SERVICES BEYOND THE CONTRACT SCOPE: The contractor shall immediately, but no later than 24 consecutive hours after discovery, notify the CO and COR (in writing) of the existence of the development of any defects in, or repairs required. DOCUMENTATION/REPORTS Reports shall include full documentation of exactly what was done. Each report shall include a unique identification of equipment and all readings and measurements found as well as analysis of the data for regulatory compliance. All completed reports are due within 10 days of final on site inspection. PERFORMANCE STANDARDS: All work must be performed by a properly trained Health Physics consultant, meeting the training requirement for Radiation Safety Officer listed in 10 CFR 35.50. All work must be performed in accordance with established radiation safety regulations, and standards. Documentation indicating qualifications (CV, certificates, etc) must be submitted to the COR prior to initiation of any work on station. APPROVAL OF CONTRACTOR PERSONNEL The VA reserves the right to refuse employment under this contract or require dismissal from contract work of any contractor employee who, by reason of previous unsatisfactory performance at the VA or for any other reasonable reason, is considered by the CO and COR to be objectionable. QUALITY ASSURANCE SURVEILLANCE PLAN This quality assurance surveillance plan (QASP) has been developed to evaluate contractor performance while implementing this SOW. The QASP provides a systematic method to evaluate the services the contractor is required to furnish. The plan is as follows: Performance Objectives Performance Threshold 1. Response by competent technician within 8 hours of contact. 2. All reports received by COR within 10 days of final on-site inspection. 3. All instrument calibrations including pick-up and drop-off are performed within 24 hours and calibrations are performed using the most current technical standard and certified as the same Number 1 customer complaints not to exceed 2 per year. Number 2 zero occurrences per year. Number 3 two occurrences per year. SURVEILLANCE The RSO and COR will evaluate the services provided by the contractor on an ongoing basis. STANDARD The contractor shall perform all work required under the terms of the SOW in a satisfactory manner. The COR shall not consider the work complete until all deficiencies have been corrected. PROCEDURES When a deficiency per the requirements of the SOW exists, it shall be brought to the attention of the COR. The COR will confirm said deficiency by inspection and record results, noting the date and time of inspection of the deficiencies for correction. The contractor shall be given four hours after notification to correct the unacceptable performance.
