Description
Technical Questionnaire with evaluation points possible for each question. No changes were made to the original questions. OFFERORS MUST ACKNOWLEDGE RECEIPT OF AMENDMENT 1 and AMENDMENT 2 BY EMAIL OR WITHIN THEIR RESPONSE. AMENDMENT 2 MAKES THE following changes to the solicitation: 1) Responses are due May 25, 2018 by 9:00 a.m. ET (CHANGED) 2) Amendment 2 Answers to questions are below. 3) Technical Questionnaire is attached with assigned points for evaluation. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, "Streamlined Procedures for Evaluation and Solicitation for Commercial Items," as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. The solicitation is being issued using simplified acquisition procedures under the authority of FAR 13.5 Test Program for Certain Commercial Items. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-97, Effective: 24 Jan 2018 (updated with Class Deviation 2018-o0007) This acquisition is set aside for Small Business only. Responses are due by 9:00 a.m. May 25, 2018 (CHANGED BY AMENDMENT 2) Only Small Businesses under the associated the associated North American Industrial Classification System (NAICS) code are invited to submit quotes. The associated NAICS code for this procurement is 541511 (Custom Computer Programming Services), with a size standard of no more than $27.5 Million (500 employees for distributors). The National Institute of Health, Office of Purchasing and Contracts, is seeking to obtain a commercially available Investigational Drug Management System for the NIH Clinical Center Pharmacy Department. The Government anticipates awarding a fixed price Purchase Order. Pre-negotiated separately priced line items may be provided in the quotation should there be additional options offered. Please indicate the availability of such upgrades or options. Contract Line Items of each period of performance must include, at least, the required system as described in the Statement of Work. ATTACHMENTS: 1) Answers to Questions/Changes to the Solicitation 2) Amended Combined Synopsis/Soliciation(including the SOW and Pricing Tables within the document), 3) Technical Questionnaire, 4) Onsite Demonstration Evaluation Form 5) Business Questionnaire Amendment 2 Amendment 2 answers vendor questions below, and breaks down technical questionnaire total possible points of 45 into points per question to establish weighted evaluation for each question or subject within the Technical Questionnaire. A. Questions and Answers 1. Vendor Question 1: Is a project plan required as part of the response? Page 16 seems to indicate yes but Technical Exhibit 1 says within 30 days of the contract award. Government Response to Vendor Questions 1: A high level project plan template is required as part of the response (Technical questionnaire 6a). A detailed project plan is required within 30 days of the contract award. 2. Vendor Question 2: Is a Quality Control Plan and Quality Assurance Plan required as part of the submission (Technical Exhibit 1). It says that it's needed if it's part of the evaluation factors but it's not listed as an evaluation factor. Please clarify is either of these plans are required as part of the submission. Government Response to Vendor Questions 2: A detailed quality assurance plan is not required as part of the submission. A quality assurance plan will be required after contract award. A high level project plan is sufficient for the submission (Technical questionnaire 6a). B. RFQ CORRECTION/CHANGES The above number Combined Synopsis/Solicitation is amended as set forth below. The date and time to submit responses is extended to May 25, 2018, 9:00 a.m. The Technical Questionnaire, as amended, provides the evaluation factors as total possible points available to earn for each answer. No other changes are made to the technical questionnaire or Combined Synopsis/Solicitation NIH-CC-18-002794. See Attached Technical Questionnaire with evaluation points possible.
