Page 1 of 2 This is a sources sought for the BT VAMC to determine the availability of potential sources (and their size classification relative to NAICS 323111 (Commercial Printing) having the skills and capabilities necessary to provide services in accordance with the attached SOW. All interested vendors are invited to provide information to contribute to this market survey/sources sought including, commercial market information. This is not a solicitation announcement. No award will be made from this sources sought. Requested information should be submitted by email to vanessa.jacobs@va.gov by 12:00pm EST May 10, 2018. Provide only the requested information below. REQUESTED INFORMATION STATEMENT OF CABABILITY: Submit a brief [ Five (5) pages or less] capability statement explaining your capability to perform services for the DC VAMC. Include past experience in performing these services for the VA, other Government (Federal or State) agency, or for a private medical facility.. BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small (b) If small, indicate if your firm your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (c) Include the DUNS number of your firm. (d) Provide your firms FSS or GSA Contract number if applicable. STATEMENT OF WORK REFERENCE: VA Maryland Health Care System (VAMHCS), VA2237 Transaction # 512-18-2-506-0213 PURPOSE: To promote health care services provided by the VAMHCS. These mailing campaigns are being initiated in an effort to increase our Veteran patient workload and enrollment, with a result of increasing our VERA reimbursement. This initiative has been implemented by the VAMHCS executive management team. GOVERNMENT REQUIREMENT: The contractor will print a full color flyer, black and white quick schedule form, business reply envelope, mailing envelope, and provide all mailing costs for the following mailing campaigns. Mailing Campaign #1: 17,002 Veterans Mailing Addresses Mailing Campaign #3: 9,783 Veterans Mailing Addresses Mailing Campaign #6: 6,307 Veteran Mailing Addresses The contractor will verify names and addresses to ensure they are valid and prepare the files for printing and mailing. Production Setup Files for Printing Address #10 Envelopes Insert Setup Insert 3 pieces into #10 Envelope & Seal Shipping & Receiving to Baltimore Post Office Printing Print Full Color Flyer & Black and White Quick Schedule Form on 24lb Paper Stock Print #10 Envelopes Print #9 Return Envelopes Provide All Postage The VA Maryland Health Care System will provide the Veterans name and address, flyer and quick schedule form for reproduction and art work for #10 and #9 envelopes. IT SECURITY/PRIVACY: Contractor providing services to VA patients shall comply with the U.S.C.551a (Privacy Act), 38 U.S.C. 5701 (Confidentiality of Claimant Records), 5 U.S.C. 552 (FOIA), 38 U.S.C. 5705 (Confidentiality of Medical Quality Assurance Records) 38 U.S.C. 7332 (Confidentiality of Certain Medical Records), Title 5 U.S.C. § 522a (Records Maintained on Individuals) as well as 45 C.F.R. Parts 160, 162, and 164 (Health Insurance Portability and Accountability Act). A Business Associate Agreement between the Department Of Veterans Affairs Veterans Health Administration, VA Maryland Health Care System and contractor is required. PERIOD OF PERFORMANCE: Performance will commence on the first day the contract is awarded.
