- REQUEST FOR INFORMATION Urology Locum Responses due 5/10/2018 by 10:00 AM EST. This is a request for information to determine the availability of contractors to meet the below need. No proposals are being solicited or will be accepted. Please see the below questions to be answered. Please do not send capabilities statements without the following questions answered. CANDIDATE MUST BE SUBMITTED WITHIN 7 BUSINESS DAYS OF AWARD. IF CANDIDATE IS FOUND UNACCEPTABLE NEW CANDIDATE MUST BE PROPOSED WITHIN 5 BUSINESS DAYS. Questions for Contractors Can you provide a Board Certified Urologist in within 7 business days of award? Can you provide a replacement candidate within 5 business days if the proposed candidate is deemed unacceptable. Is your proposed candidate Board Certified in American Board of Urology? Can you provide confirmation of a full range of Surgical Ability List of such abilities. Does your candidate have a current unrestricted license that has never been revoked or has NO Sanctions/Adverse Actions? Has your candidate ever had privileges suspended? Does your candidate have 3 or more NPDB reports? Does your candidate have a malpractice claim over $500,000, or 3 claims totaling $1,000.000.00? The Contractor shall furnish all personnel to provide services necessary to perform onsite Urology Physician Services to eligible beneficiaries of the Department of Veterans Affairs Medical Center, Lebanon VA Medical Center (hereinafter referred to as VAMC). The contractor s physician(s) care shall cover the range of Urology services as would be provided in a state-of-the-art civilian medical treatment facility and the standard of care shall be of a quality, meeting or exceeding currently recognized national standards as established by: American Urological Association Guidelines: https://www.auanet.org/education/aua-guidelines.cfm Place of Performance: Services shall be provided on site: Lebanon VA Medical Center 1700 South Lincoln Avenue Lebanon, PA 17042 Pricing Instructions: This will be a firm fixed price contract. Invoicing will be done monthly unless otherwise stated. One FTE is defined by VA as a minimum of 80 hours every two week per year and does not include holidays. ITEM NUMBER DESCRIPTION OF SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 Board Certified /Board Eligible Urology Physician Services to include: Outpatient Clinic Services for Medical Urology, Outpatient Clinic Services for Surgical Urology, Inpatient Urology Consultations for Medical Care and/or Surgical Procedures for the below timeframes and schedule.
