This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-96 effective November 6, 2017. The associated North American Industrial Classification System (NAICS) code for this procurement is 334516 with a small business size standard of 1,000 employees. The Department of Veteran Affairs, SAO West, Network Contracting Office-22, 4811 Airport Plaza Drive, Suite 600. Long Beach, CA 90815 is seeking to purchase Affirm Test Kits. All interested companies shall provide quotation(s) for the following: Supplies Item # Description/Part Number* Qty 1 BD Affirm VPIII 24 test kit/446252 50 Statement of Work 1. Introduction The purpose of this Statement of Work (SOW) is to outline the tasks and responsibilities of the vendor in relation to the purchase of Affirm kits for the VA Greater Los Angeles Healthcare System Microbiology section. 1.1 Background The VAGLAHS at 11301 Wilshire Blvd, CA 90073 is part of the Desert Pacific Healthcare Network (VISN 22), and is one of the most diversified tertiary health care providers in the VA. It is a teaching hospital, providing a full range of patient services, with state-of-the art technology as well as education and research. The BD MicroProbe Processor is already on site within the Laboratory. The instrument and the kits used to perform the tests have already been validated. 2. Tasks 3.1 General Requirements Affirm test kits must be compatible with our current system, BD Affirm VPIII. The reagents are to be ordered on an as needed basis. Reagents must ship directly to the laboratory, B500/RM1294 3.2.3 Hours of Operation Normal tour-of-duty hours at the VAGLA is 7:30 am to 5:00 pm, Monday through Friday, excluding Federal holidays. There may be instances in which the vendor s workday may be outside of this period. The period of performance must be one year from award date. Supplies shall be ordered on an as needed basis. The contractor shall deliver all line item(s) to VA Greater Los Angeles Healthcare System (VAGLAHS) 11301 Wilshire Blvd. Los Angeles, CA 90073. The government will evaluate information based on the following evaluation criteria: (1) Technical capability and (2) Lowest price. Technical acceptability shall be established by review of each submitted quote by the designated technical evaluator/s and verification that any equal to items meet the statement of work included in this solicitation. Submissions to this solicitation must show clear, compelling and convincing evidence that all equal to items meet all the requirements in the statement of work.
