J SYNOPSIS OF PROPOSED CONTRACT ACTION - SOLE SOURCE CONTRACT-PREVENTIVE MAINTENANCE ON STORZ ENDOSCOPES Solicitation Number/FBO Notice: 36C24518Q9030 Agency: Department of Veterans Affairs Office: Washington, DC Medical Center Location: Washington, DC Response Date: Monday, May 7,, 2018 @ 2:00pm Set Aside: N/A Classification Code: J Maintenance, repair and rebuilding of equipment NAICS Code: 811219 Synopsis: The Dept of Veterans Affairs, in accordance with FAR 6.302-1, intends to make a sole source award to a firm with the capability of providing preventive maintenance, service, repairs, and exchanges to Karl Storz endoscope located at the Washington DC VA Medical Center The anticipated source is Storz-Karl Storz endoscopes, 2151 Grand Ave, El Segundo, CA 90245-5017. This is not a formal solicitation or request for quote. This synopsis of intent is to give notice of the Government s intent to make a sole source award. Contractors who can provide the Dept. of Veterans Affairs with the required services are invited to electronically submit an affirmative written response indicating a bona fide ability to meet this specific requirement. Specifications for this requirement are as follows: The Contractor shall provide all labor, personnel, equipment, tools, materials, supervision and other items and services necessary to repair or exchange the Karl Storz instruments, electromechanically devices, and both rigid and flexible endoscopes at the Washington DC VA Medical Center. The contractor shall also provide unlimited service and maintenance to support the Storz products. The Contractor shall also provide unlimited replacement parts to return the Storz products to normal operating conditions. If parts are backordered, the Contractor shall provide loaner products. Service provided shall be no fault in that all repairs necessary fall under the scope of this contract regardless of cause of defect or damage. Field Service Technician visits shall be at no charge to the government. Technology upgrades shall be included at the time of exchange or repair. Additional training will be provided upon customer request or product installation. Damage reports and field service reports shall be provided for every repair/ exchange. Repairs and exchanges of non-expendable equipment in the DC VAMC inventory shall be reported to Biomedical Engineering and contractor shall provide a service report. Biomedical Engineering will be the manager and administrator of the service contract. Responses should appear on company letterhead and include, at minimum, affirmation of active registration in the System for Award Management (SAM), your Data Universal Number System (DUNS) number, any applicable processed credentials and qualifications, and any applicable pricing data or information. All responses should include a Point of Contract (POC), telephone number, and electronic mail address (email). Additionally, respondents should indicate whether they are a large business, small business, small disadvantaged business, 8(a) concern, women-owned small business, HUBZone, service disabled veteran-owned small business, or qualify as socially or economically disadvantaged and whether they are US or foreign owned. Responses to this notice will be used to determine the availability of this type of service. All responsible sources may respond to this synopsis and all responses will be considered by the agency.
