The Department of Veterans Affairs Jesse Brown VA Medical Center located at 2030 West Taylor in Chicago, IL 60612 is seeking to purchase a Conveyor Belt with Installation and Kitchen Preparation Items needed for mixing, chopping and grinding food to help make nutritious meals for Our Nation s Veterans within the Jesse Brown VA Medical Center. (Line item 0001) The Conveyor and Belt equipment is required to assist in providing clean and sanitary meals to our patients; approximately 750-1000 meals per day prepared by our service. The current conveyor is at the end of its life cycle. This conveyor belt configuration is essential for us to provide fresh and nutritious meals to Veterans who have various feeding and food requirements. The Delivery Location for this conveyor belt will be at Taylor Dock, Food and Nutrition bldg. on the Jesse Brown VA Medical Center property grounds. Installation location will be: 3rd Floor Damen, Nutrition and Food bldg., Service Kitchen Room # 3495, Jesse Brown VA Medical Center, 2030 West Taylor, Chicago, IL 60612-4223. Location of area and installation will be by main loading dock area, and then by elevator to reach the 3rd floor area where installation will occur. Training on this conveyor equipment is needed and should occur within days of complete installation of conveyor belt, to enable immediate use of conveyor by all employees. Upon scheduling of installation of new conveyor, all employees will be scheduled to have required training on same/next business day that installation is finished and complete. This scheduling will be coordinated by Department Manager; training must be at least 2 separate times for one day to accommodate all shift employees. 2. (Line item 0002), the Installation of the Conveyor, MUST be performed by the OEM, or an authorized distributor who can confidently install the new conveyor and test this conveyor and ensure its absolute brand-new working order. This OEM or authorized distributor must also be confident in the deinstallation of an older model conveyor, and understand the many issues with installation/set-up that can show up during this time and know how to find ways to solve these problems. This installation of Conveyor job will also include the de-installation and complete breakdown of the current standing conveyor and attachments, the transport and disposal of the existing current conveyor belt, the unpacking and set-up of the new conveyor belt and attachments, the complete setup of new conveyor belt and attachments, the complete installation of new conveyor belt, and an after action conveyor test run, whereby the vendor can ensure that the conveyor is in new working order and has no defects upon inspection and can ensure the new working order of the new conveyor. 3. (Line item 0004, 0005 and 0006) Function of the Induction Chargers Thermal Pellet Base/Underliner Equipment: To insulate and hold appropriate temperature of room service meals for many Veterans. The Dinex induction units and the Dinex heated bases must be of the same brand as they function together and not independently; one will not work without the other. Please see posted J and A requesting Dinex Brand Name items that details why this name brand is required over other brands. The requested warranty on these items is essential to maintain constant use and good working order. The delivery/task order period of performance is as soon as possible after contract award has occurred. Delivery shall be provided no later than 45 days after receipt of order/award of contract. All quotes are requested to be submitted to CO as FOB destination. The contractor shall deliver ONLY line item(s) 0001 to 0002 to Taylor Dock, Food and Nutrition bldg. on the Jesse Brown VA Medical Center property grounds. Delivery and Installation location will be: 3rd Floor Damen, Nutrition and Food bldg., Service Kitchen Room # 3495, Jesse Brown VA Medical Center, 2030 West Taylor, Chicago, IL 60612-4223. The contractor responsible for bringing this specific equipment will need to first check in with the main gate and warehouse, before proceeding directly to the Food and Nutrition Department on Jesse Brown VA Medical Center Grounds to unload equipment at loading doc. Coordination must be given in advance of any conveyor movement from the vendor moving the new conveyor on to the VA Medical Center Grounds and into the Food and Nutrition Department. The government will evaluate information based on the following evaluation criteria: (1) technical capability factor for meeting the requirement, (2) past performance (3) price, and (4) days to complete project after contract award is made. Award will be made in accordance with FAR 13.106-2(b)(3), by using Comparative Value, which will help determine Best Value for the Government. This is determined by first determining acceptability of quote by ensuring all requirements are noted within. Comparative Value is the act of comparing two or more quotes in response to a Request for Quotes. The item-by-item comparison is performed by comparing each offer to one another to determine which provides the best benefit to the Government. The Government reserves the right to select a response that provides the best benefit to the Government that meets all requirements. Even though quotes may exceed the stated requirement, the Government is not requesting or accepting alternate quotes; each response must respond to this solicitation requirement statement. Considering Best Value, Relative importance of each factor is equal with regards to other factors. This award will NOT be based on LPTA (Lowest Price Technically Acceptable) or trade-off process. The Technical Capability Factors requires all quotes submitted to also have the same 17-line items that matches this notice, and the required statement regarding terms and conditions of contract.
