- RAYMOND G. MURPHY VA MEDICAL CENTER Ambulance Stretcher and Stair chair STATEMENT OF WORK (SOW) Raymond G. Murphy VA Medical Center (Albuquerque, NM) request one power ambulance cot and one tread driven stair chair installed for the REdI Team. Background. The procurement of one ambulance cot and stair chair for the REdI Team which will become an integral part of our campus Emergency Response Team which will allow safe lifting and moving of patients from the scene to the emergency department and will be in accordance with MCM 118-50 which speaks to staff not lifting greater than 35 lbs./ 15.9 kg of patient weight. Scope. The overall scope of work is the procurement of one ambulance cot and stair chair for the REdI Team. The selected equipment manufacturer will furnish all necessary equipment. Vendor shall provide all labor, material, tools, parts, equipment, operating and/or Technical/maintenance manuals, training, delivery, and accessories necessary to safe operation of the ambulance cot and stair chair. IEC 60601-1 and IEC 60601-1-2: This certification includes that the power cot conforms to industry standards for mechanical and electrical safety for medical electrical devices, as well as electromagnetic compatibility and immunity. BS EN-1789 clause 4.5.9: This is a European dynamic crash test which subjects a 50th percentile dummy to a nominal 10g deceleration for a minimum of 50ms. Following the test, there shall be no sharp edges or danger to the safety of persons in the road ambulance. Powder-coating of the entire aluminum frame (including the patient handling surfaces), thus eliminating aluminum oxidation throughout the cot. Ambulance cot must have approx. 6 x 2 size with sealed bearing casters. Ambulance cot is power washable. Ambulance cot weight capacity of 700 lbs / 317.5 kg Ambulance cot, when unloading with the manual release handle will utilize hydraulic dampening. Thus, the cot will not abruptly jar the patient or operator. Ambulance cot will need to provider a load height of 36 /.9144 m and is operator adjustable to match the deck height of individual ambulances. Ambulance cot will provide controls and lifting bars at two different heights to provide optimum ergonomics to most operator heights. The battery is located on the foot end of the ambulance cot The ambulance cot legs power retracts in 2-3 seconds which will speed load time of the patient. The foot end of the cot contains a large battery indicator light which indicate battery level. A warning is given, providing the operator the time to change the battery before full depletion of power. The ambulance cot will also have a foot end mounted usage meter. This will determine preventative maintenance checks. Seat assembly is mounted to the main frame and configured to pivot about on a first pivot access. High visibility powder coated frame. Lightweight, rugged aluminum construction. Stair- TREAD system or equal Compact storage User manual and maintenance manual CLIN 00003 Installation: Contractor shall be responsible for a complete installation. Installation to be completed by the contractor shall take place during normal working hours 8:00am-4:00pm local time unless otherwise agreed upon with the POC. Contractor shall configure the system proposed to meet those functional requirements. Contractor shall pretest the entire system. After pretesting the system, the Contractor shall notify the COTR that the system is ready for acceptance testing. Contractor shall provide training on the use and operations of the equipment. Contractor shall provide two (2) copies of the Operating and /or Maintenance manuals, to include electronic copies. 7. Security Requirements The C&A requirements do not apply, and that a Security Accreditation Package is not required: 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). None 9. Other Pertinent Information or Special Considerations a. The ambulance cot shall carry a (3) year power train, and (2) year bumper to bumper warranty. The stair chair shall carry a (1) year parts, labor and travel. Continued training and in-servicing will be available at no cost for the life of the equipment. b. Inspection and Acceptance Criteria. Upon completing installation of the system, the Contractor shall pretest the entire system. After pretesting the system, the Contractor shall notify the POC that the system is ready for acceptance testing, and that it meets all requirements as specified. Submission of this notification of system readiness shall be accomplished by the contractor prior to the beginning of the scheduled Government Acceptance Test. 11. Place of Performance. Installation and training will be performed within REdI Team Training Center located on the basement level of the Raymond G. Murphy VA Medical Center. Installation to be scheduled with POC. 12. Period of Performance. a. Delivery 4-6wks ARO. b. Installation: Normal hours of operations at the VAMC are Monday through Friday from 8:00 am to 4:30 pm, excluding holidays. All work will be performed during normal hours of coverage unless requested or approved by CO or his designee.
