Description

THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The Cheyenne VA Medical Center located in Wyoming and the Network Contracting Office (NCO) 19 are performing market research to determine if there is sufficient number of qualified providers/firms for courier services. Socio-economic precedence are as follows: (1) Service Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; or (3) Small Business to set aside GENERAL REQUIREMENTS Firms responding shall specify they can meet the required specifications: Provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items necessary to perform courier services to transport laboratory specimen, pharmaceutical supplies, prescriptions, office supplies, mail, small medical equipment from the Bio-Med Department, clean and soiled laundry, medical supplies for clinical operations, and prosthetic supplies for clinical operations. Courier services shall transfer said items, or any of them, to and from the Cheyenne, Wyoming V.A. Medical Center, located at 2360 East Pershing Blvd., Cheyenne, WY, 82001 to and from the Multi Specialty Outpatient Clinics ( MSOCs ) located in Loveland, Colorado, Fort Collins Colorado, Sidney, Nebraska, and Rawlins, Wyoming. The address for the MSOC located in Loveland, CO is 5200 Hahns Peak Drive, Loveland, CO 80538. The address for the clinic located in Fort Collins, CO is 2509 Research Blvd., Fort Collins, CO 80526. The address for the clinic located in Sidney, NE is 1116 10th Ave, Sidney NE 69162. The address for the clinic located in Rawlins, WY is 1809 East Daley St., Rawlins, WY 82301. All interested parties who can meet the requirements stated above should respond to this announcement via email to bai.perney@va.gov no later than Tuesday, May 4, 2018 at 2:30 PM (Mountain Standard Time). Interested parties should provide the following: Company Name, Company Address, Point of Contact, phone number and email address, DUNS number and capability statement for the services as stated above. Additionally include; two (2) customer references to which you have provided similar services, and your company business size indicate which small business category for which you qualify. THIS IS NOT A REQUEST FOR PROPOSAL. NAICS Code 492110 is applicable to this acquisition, and the business size standard for a contractor is 1,500 employees. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the System Award Management System (SAM) database prior to award of a contract.
