Description

THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The VA Montana Healthcare System at Fort Harrison and the Network Contracting Office (NCO) 19 are performing market research to determine if there is sufficient number of qualified providers/firms for courier services. Socio-economic precedence are as follows: (1) Service Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; or (3) Small Business to set aside GENERAL REQUIREMENTS Firms responding shall specify they can meet the required specifications: The VA Montana Healthcare System at Fort Harrison requires same day delivery service of small packages throughout Montana. Fort Harrison is located five miles outside of Helena, Montana city limits. The size of the laboratory specimen boxes will average 22 by 22 square or less in size, some return boxes containing supplies may be somewhat bigger. Two to six boxes containing laboratory specimens will require transportation to the Ft. Harrison campus from each of six (6) VA Community Based Outpatient Clinics (CBOC s) daily, Monday through Friday. In addition, medical supplies requiring same day delivery may also be transported to Ft. Harrison from the clinic sites. Boxes being returned to the clinic sites (empties and some containing supplies) will be picked up in the afternoon/evening and delivered back to the clinics the next day; Friday s pickups will be delivered back to the clinics the following Monday. The following distances are taken from State of Montana city-to-city mileage guide and are for information purposes only. Actual mileage from CBOC s to Fort Harrison may vary: Anaconda 83 miles Bozeman 95 miles Great Falls 89 miles Kalispell 197 miles Missoula 116 miles Lewistown 129 miles Havre 204 miles Hamilton 158 miles All interested parties who can meet the requirements stated above should respond to this announcement via email to bai.perney@va.gov no later than Tuesday, May 4, 2018 at 2:30 PM (Mountain Standard Time). Interested parties should provide the following: Company Name, Company Address, Point of Contact, phone number and email address, DUNS number and capability statement for the services as stated above. Additionally include; two (2) customer references to which you have provided similar services, and your company business size indicate which small business category for which you qualify. THIS IS NOT A REQUEST FOR PROPOSAL. NAICS Code 492110 is applicable to this acquisition, and the business size standard for a contractor is 1,500 employees. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the System Award Management System (SAM) database prior to award of a contract.
