Description

Notice of Intent to Sole Source Requirement The Department of Veterans Affairs, VA Healthcare System, VISN 10, Contracting Activity intends to negotiate a sole source contract under Simplified Acquisition Procedures and the authority of (FAR 8.405-6(a)(1)(i)(A) An urgent and compelling need exists, and following the procedures would result in unacceptable delays); for the following items listed below. The associated NAICS code for these Supplies is 339112. The vendor, Veterans Healthcare Supply, 13949 Alvarez Road, Suite 300, Jacksonville, FL 32218-9433. Description of equipment requirements: 10 Each, Medtronic Infuse Bone Graft Large II Kit, 10ml Vial Water Infused (MPN): 7510800. A firm fixed price contract with no option years is anticipated and shall be awarded. It is the Government's belief that only the proposed candidate possesses the required capabilities to successfully meet this requirement. It is the Government's intention to solicit and negotiate with only this source. This is not a Request for Proposal; this is a Notice of Intent to Sole Source requirement. DEPARTMENT OF VETERANS AFFAIRS Justification for Single Source Awards IAW FAR 13.106-1 For Over Micro-Purchase Threshold but Not Exceeding the SAT ($150K) Acquisition Plan Action ID: VA250-18-AP-7686 Contracting Activity: Department of Veterans Affairs, VISN 10, Richard L. Roudebush VA Medical Center, 1481 West 10th Street, Indianapolis, IN 46202. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The Service Submitted Purchase Request (VA2237) 583-18-3-943-3340 with an estimated procurement value of $59,361.90, which has been set aside to purchase 10 ea., Infused, Large, Bone Graft Kits (MPN): 7510800. The INFUSE ® Bone Graft stimulates the recruitment and differentiation of bone-forming cells inducing new bone formation or healing existing bone. It contains a recombinant version of bone morphogenetic protein-2 (rhBMP-2), which is known to be upregulated in the bone healing process assisting in bone regeneration. This item is required to be delivered to the facility on or before April 18, 2018. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The INFUSE ® Bone Graft stimulates the recruitment and differentiation of bone-forming cells inducing new bone formation or healing existing bone. It contains a recombinant version of bone morphogenetic protein-2 (rhBMP-2), which is known to be upregulated in the bone healing process assisting in bone regeneration. This item is proprietary to the manufacturer, Medtronic and Veterans Healthcare Supply is the SDVOSB authorized distributor of this product. This is an emergent/special procurement and is required for a patient s scheduled surgical procedure. Without these products, patient care is greatly affected resulting in the patient s surgical procedure being cancelled and rescheduled. Description of market research conducted and results or statement why it was not conducted: Market Research was conducted on a minor scale due to this procurement being an emergent order. However, the following was reviewed: FPDS-NG, GSA, MedSurg Online Catalogs, NAC, and Sam - for vendors with related items without success. A google search was also conducted only to provide ineffective results. Note: Veterans Healthcare Supply is a small disabled veteran owned company. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
