SYNOPSIS: An employee (Dietitian) needs a Live Reader one (1) Saturday every six weeks when she covers the inpatient units at the Baltimore VAMC. The Live Reader will use the dietitian s account to access the medical record, will navigate to appropriate patient charts per the dietitian s direction, and will assist with gathering information from patients medical records for her to effectively write notes the same day patient is seen. This will require extensive searching of the medical record to gather the information needed, as well as reading the information aloud to the dietitian. After the dietitian provides care to the patient, the reader would transcribe the employee s real time dictation into a note template.   The employee will review the note and electronically sign it.   Every note will state Note scribed by (name and title of Live Reader).This will allow employee to meet documentation and productivity standards in a timely manner. The Reader is required to be proficient at the English language, as well as possess excellent reading, speaking, and typing skills. As this dietitian is required to have this type of support, the contract company needs to provide a backup Live Reader who can fill this role if the primary reader is unavailable. A Business Associate Agreement will be established between the VA Maryland Healthcare System and the Contractor which establishes requirements for the use of PHI (Protected Health Information) in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
