Description
SOLICITATION: 36C79118R0014 REQUIREMENT: 3rd Party Insurance PURPOSE OF AMENDMENT: The purpose of this amendment is to make the following changes to the solicitation: Post questions and answers received in regards to the current RFP 36C79118R0014 3rd Party Insurance. The answers are in red. Update Attachment 1 Run Loss Reports. Post questions and answers Has your current insurance carrier issued a non-renewal or conditional renewal notice for the 2018-2019 renewal term?   If yes, what reason is listed on the notice? No. Can you provide currently valued (dated within  90 days of the 2018 effective date)  insurance company loss runs for at least the past  five (5) years (2013-2018 policy terms)?  Please see the revised Attachment 1 Run Loss Report. If the department has any auto losses greater than $25,000, please provide a full description of the loss? Please see the revised Attachment 1 Run Loss Report. Are random drug and alcohol tests performed on all drivers? No. Does the department have a formal fleet safety program? Each VA facility complete licensing check but nothing is mandated from PM&R. Does the department have a formal driver selection program including a review of MVRs at least annually? Each VA facility has the discretion to manage the selection process. Does the department have a formal procedure to investigate any accident and perform post accident reviews? If the accident occurs on VA grounds, it is investigated by VA police. If the accident occurs off of VA grounds the investigation is completed by the local police department. Local police departments may perform post accident reviews but this is not done at a national level. Does the department provide drivers with written rules for withdrawal of driving privileges for serious driving violations  such as but not limited to  DUI, Reckless driving, speeding more than 20 mph above the posted limit? Each VA facility has local rules that apply. Does the department have a policy against personal use of the vehicles by employees? Yes, employees are prohibited from using the vehicles for personal use. Can you provide a list of driving instructors for all vehicles with their name, date of birth, state licensed, and their driver license number? Yes, this information will be disclosed at time of award to maintain driver s privacy. Are there any controls in place for selecting program participants that have had a poor driving history? Yes,   we ask for verification that they have not: a) had a conviction for an alcohol and/or drug related driving offense, b) refusal to submit to a Blood Alcohol Content (BAC) test, c) had a conviction for reckless driving, d) any combination of three or more moving violations At Fault Accidents, or Preventable Accidents within the most recent three years, e) suspension, revocation or administrative restriction within the last three years, f) leaving the scene of an accident as defined by state laws, g) at fault in a fatal accident, and h) felony committed involving a vehicle How is access to the vehicle controlled by the VA? Are the participants allowed to take the vehicles out unaccompanied or is there an instructor riding along at all times? Patients are not allowed to take the vehicles unaccompanied.   There is always a VA driver instructor in the vehicle with the exception of when the vehicle is used for a State behind the wheel test when the Veteran is accompanied by a State motor vehicle department employee. What is the training radius of operation around the VA locations? This question is not answerable. Please explain the driver selection process. Are there any disqualifiers for a potential participant to enter the program? Please see question K above. Are participants allowed to operate the vehicles while under the influence of pain management medications? Veterans may be on whatever medicine is prescribed. Is a Motor Vehicle Report (MVR) obtained prior to enrolling a participant in the program? No.   How are the vehicles modified, and are the modifications installed by an expert? Vehicles are modified and this is completed per acquisition requirements. What is the current deductible or retention on the expiring policy? What is the desired deductible or retention for the desired new policy?   What is the premium amount on the expiring policy? Who is the incumbent contractor? II.)
