Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This combined synopsis/solicitation is issued as a Request For Quotations (RFQ). NAICS: 811219 Other Electronic and Precision Equipment Repair and Maintenance. Size Standard: $20.5M This combined synopsis/solicitation is being issued as a Service Disabled Veteran Owned Small Business Set-Aside. Public Law 109-461, 38 USC Sec. 8127 provides SDVOSB set aside authority to the Department of Veterans Affairs. Businesses must be registered and verified in the VIP database at Vendor Information Pages (VetBiz). Award will be made to the responsive offeror whose offer is the lowest priced and conforming to the solicitation considering only price and any price-related factors specified in the solicitation. Place of Performance: VA Greater Los Angeles Healthcare System VA West Los Angeles Medical Center Building 501 11301 Wilshire Blvd. Los Angeles, CA 90073 Period of Performance: 30 days from the date of award The Department of Veterans Affairs Greater Los Angeles Healthcare System (VAGLAHS) has a requirement to repair medical and dental air compressors per the Statement of Work. The existing equipment is in poor condition and in need of upgrades. All services will be performed in strict adherence to the Statement of Work (attached). List of Attachments: Solicitation 36C26218Q0509 Statement of Work, dated April 12, 2018 Applicable Service Contract Wage Rates Los Angeles County WD 15-5613 Facility Map West Los Angeles Campus Site Visit: A site visit will be conducted on Tuesday April 24, 2018 at 10:00 A.M. PST. Attendance is NOT mandatory, but highly encouraged at the VA Greater Los Angeles Healthcare System. Participants are asked to meet outside of Building 501 located on the south side of the West Los Angeles Medical Center, right behind the main hospital building. All offerors MUST RSVP for this site visit. Please RSVP via email to Contract Specialist, James Barber at james.barber@va.gov NLT Monday April 23, 2018 at 1:00 P.M. PST. The following clauses apply: FAR 52.212-1 - Instructions to Offerors applies to this acquisition Provision Addenda: All quotes shall include the following minimum information: Cage Code, Duns Number, Taxpayer I.D. No., Payment Terms, and/or Discount Terms, Offerors Reps and Certs, Point of Contact, Phone, Fax, Email and Proposed Delivery Schedule. Prospective vendors must be registered in the System for Award Management (SAM) database prior to contract award. Lack of SAM registration shall be a determining factor for contract award. Prospective vendors should visit the SAM website at http://www.sam.gov to register. FAR 13.106-2 Evaluation of Quotations or Offers - Commercial Items: Contract award will be made to the responsive offeror whose offer is the lowest priced and conforming to the solicitation considering only price and any price-related factors specified in the solicitation.
