Description

The Amarillo VA Health Care System (AVAHCS) is seeking Service Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) sources capable of meeting the requirement listed below. The acquisition will be accomplished using commercial item procedures in accordance with FAR Part 12. NOTE: TO QUALIFY SDVOSB AND VOSB VENDORS MUST BE CERTIFIED IN THE VA VENDOR INFORMATION PAGES (VIP) DATABASE. The North American Industry Classification System (NAICS) is 334510. Any SDVOSB or VOSB firms who wish to identify their interests and capability to provide this product must provide product specifications, performance and delivery information by notifying the Contract Specialist no later than NOON PM Central Time, April 23, 2018. Notification shall be e-mailed to Robyn Nussbaum, at robyn.nussbaum@va.gov. Any vendor who responds to this Notice must provide credentials to provide automatic slide strainers in accordance with the Statement of Work. All information received in response to this Notice that is marked as proprietary will be handled accordingly. In accordance with the Federal Acquisition Regulation, responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this Notice. SCOPE: The Amarillo Veterans Affairs Health Care System (AVAHCS) Department of Pathology & Laboratory Services (P&LMS) requires 2 replacements for the current methodology of preparing hematology slides for the Hematology department. The methodology currently used for the hematology slide staining will no longer be supported by the manufacturer. This procurement will update the present equipment and bring technical advances up to date, will provide consistency for all sample types of slides since it is not technique dependent and allows the technologist to walk away to perform other testing. The vendor shall provide, in accordance with all applicable federal, state and local regulations, laws, and ordinances, and in accordance with the specifications outlined for accreditation certification if applicable, equipment and supplies as negotiated on GSA contracts to enable AVAHCS to accurately and efficiently identify infectious agents in cells, body fluids, and tissues. Vendor Responsibilities The Vendor shall provide a price for 2 automated slide stainers for the Hematology department located at: Amarillo VA Health Care System Lubbock CBOC 6010 Amarillo Blvd., West 6104 Ave Q South Drive Room 2256-28 AND Room 700-98 Amarillo, TX 79106 Lubbock, TX 79412 Attention: Laboratory Attention: Laboratory The Vendor shall provide an automated slide stainers to meet the following requirements: 100/230V External dimensions (H x D x W inches) 9 x 18.5 x 21.5 Operating Temperature: Room temperature Weight is less than 45 pounds Operator and maintenance friendly Self-contained waste drawer or the ability to use tubing to drain into plumbing Carrier must be continuous load/feed using a spiral gear Capable of staining 55+ slides per hour Platens provides uniform stain from field to field and slide to slide Automatically stains slides using peripheral blood and/or bone marrow smears Meets requirements of TUV and UL Power cord Slide collection drawer Stained slide dryer Alert to warn of low reagents All Consumables can be sent with the slide stainers upon installation (see table below) Line No. Description of Consumables Unit Est Usage Per Year 1 Cannula set 3/pk 2 2 Pump Tube set 3/pk 2 4 Under platen Tubing 3/pk 2 Delivery/Installation Vendor shall provide one set of operator s manuals per location site. The AVAHCS shall prepare the site at its own expense and in accordance with the specifications furnished by the Vendor. The Vendor shall incur freight charges and ship the automated slide stainers to the addresses located at: Amarillo VA Health Care System Lubbock CBOC 6010 Amarillo Blvd., West 6104 Ave Q South Drive Room 2256-28 AND Room 700-98 Amarillo, TX 79106 Lubbock, TX 79412 Attention: Laboratory Attention: Laboratory The performance period shall commence on the date of installation with regards to Maintenance/Warranty Service. Billing a. Vendor shall send invoice for the 2-automated slide stainers to: Amarillo Veterans Affairs Health Care System 6010 Amarillo Blvd., West Amarillo, TX 79106 Attention: Logistics Licensing and Accreditation The Vendor shall have all licenses, permits, accreditation and certificates required by law and or regulatory compliance. Personnel assigned by the Vendor to perform these services covered under this contract shall be licensed in a State, Territory, or Commonwealth of the United Sates or the District of Columbia.
