- Wage Determination Flathead CSE Exhibits RFQ Flathead Common Stand Exam This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 of the Federal Acquisition Regulations (FAR), as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation and incorporated provisions and clauses are those in effect through the most current Federal Acquisition Circular 2005-96. The clauses and provisions referenced in this solicitation can be found in full text format at http://farsite.hill.af.mil/ or www.acquisition.gov. This procurement is a 100% small business set-aside and the small business size standard is $7.5 million. The North American Industry Classification (NAICS) code for this acquisition is 115310. Solicitation number 12034318Q0132 is a Request for Quotation (RFQ) for Region One Intensive Common Stand Exam for the Flathead National Forest. The awarded purchase order will be a firm fixed price. One award will be made to the Quoter whose quote, as determined by the Contracting Officer is most advantageous to the Government, considering but not limited to factors such as: past performance, capability to accomplish this type of work, quality control, safety plan, familiarity with Region 1 Intensive Common Stand Exams, and price. The Experience Questionnaire will be used as part of this evaluation. Period of performance will include a base year (12 months) to begin at time of award plus 1 Option year. Option years are exercised at the discretion of the Government in accordance with FAR 17.207. The FAR provision at 52.212-1, Instructions to Offerors - Commercial Items, and 52.212-3 Offeror Representations and Certifications - Commercial Items applies to this acquisition. The FAR clauses 52.212-4, Contract Terms and Conditions - Commercial Items, and 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, apply to this acquisition. Prospective awardees Must be registered in System for Award Manag ement (SAM) to be awarded a Federal Contract. The following items will need to be submitted; 1. Schedule of Items, containing quoted amounts 2. Experience Questionnaire 3. Quality Control Plan 4. Work Plan and Rate of Progress Schedule 5. Safety Plan Quotes must be received by May 3, 2018, 1:00 PM, MST at the Lolo National Forest Supervisor's Office, Attn: Theresa Peterson, AQM, 24 Fort Missoula Rd., Missoula, MT. 59804.
