Description

Greetings, This is a Request for Quote. Can you please provide a quote for Birmingham VA Medical Center? 36Q24718Q0424 VENTILATORS: See Price and Cost Schedule on Solicitation Combined Synopsis/Solicitation for the Birmingham VA Medical Center. See attached solicitation document for details of this requirement. This is a RFQ for Ventilators for the Birmingham VA Medical Center.   This requirement is to promote and provide Full and Open Competition in accordance to Far Part 6.100. Competitive procedures available for use in fulfilling the requirement for full and open competition in accordance to Far Part 6.102(b) Competitive proposals. The Government reserves the right to award a single contract for each site on an all, or none basis. The Government intends to make award to the eligible, responsible offeror(s) whose offer(s) is the most advantageous to the Government considering price, technical factors and past performance factors. Offerors shall bid on all items as partial bids are unacceptable. Offerors must include the following information in the quote submission: 1. Contractor point of contact including address, phone number, fax number and email address. 2. Contractor CAGE number and DUNS number. 3. Minimum and maximum ordering limitations, if applicable. 4. Prices per line item or service. 5. Quantity price breaks. 6. All questions should be submitted in writing. 7. GSA / FSS Schedule Number EVALUATION FACTORS Lowest price technically acceptable process is appropriate for this award as the best value is expected to result from selection of the technically acceptable proposals that meet or exceed the acceptability standards for non-cost standards with the lowest evaluated price. Preference shall be given to a Service Disable Veteran Owned Small Business (SDVOSB) offerors in accordance with FAR 19.1401(b) when a large business or small business will have essentially the same bottom-line price. All vendors must meet the eligibility requirements in 38 U.S.C.8127(e), (f) and VAAR subpart 819.7003 and be listed as verified in the Vendor Information Pages (VIP) database.
