VENTURA, CA 93003-7398 Products: P/N T18-0067M, Anatomical Bone Model 1 Each P/N T18-0067LF, Left Fossa Component 1 Each P/N T18-0067LM, Left Mandibular Component 1 Each P/N T18-0067RF, Right Fossa Component 1 Each P/N T18-0067RM, Right Mandibular Component 1 Each P/N CHG020, Charge, Bilateral TMJ Implants (bundled cost) 1 Each P/N DPYY-NNN, Protomed DigiGuide Product 2 Each These surgery parts were chosen because in the surgeon's opinion, it meets specific needs and requirement of the patient's condition and will provide the desired surgical outcome. These are specific products that are requested by the surgeons. These products are considered all or none and in the best interest of the patient. No telephonic request will be honored or returned.
