Attachment 6 - BPA Pricing Fill-in Table Attachment 5 - Reps and Certs Attachment 4 - Canine Delivery Instructions Attachment 3 - Delivery Tracking Attachment 2 - Medical Record Delivery Instructions Attachment 1 - Medical Requirements and Evaluation BPA Terms and Conditions This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 70T02018Q9NOTD408 and is issued as a request for quotation (RFQ). Quoters are requested to review the attached BPA Terms and Conditions document, which is the solicitation document itself, as well as its attachments: (1) Medical Requirements and Evaluation; (2) Medical Record Delivery Instructions; (3) Delivery Tracking; (4) Canine Delivery Instructions; (5) Reps and Certs; and (6) BPA Pricing Fill-in Table. Please read all documents carefully. The RFQ and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97. The RFQ provides for full and open competition. The NAICS code is 112990. List of contract line item number(s) and items, quantities and units of measure: See attached BPA Terms and Conditions document. Description of requirements for the items to be acquired: See attached BPA Terms and Conditions document. Date of Delivery: See attached BPA Terms and Conditions document. Place of delivery and performance: See attached BPA Terms and Conditions document. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition, however no alternative terms and conditions or commercial items outside those specified in the attached BPA Terms and Conditions document will be considered. The provision at 52.212-2, Evaluation -- Commercial Items, is not used in this solicitation. Please see the attached BPA Terms and Conditions document for the evaluation procedures to be used. Quoters shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its quote if it does not include the statement as dictated on page 37 of the BPA Terms and Conditions Document. The number of BPAs awarded will be at the Government's discretion, but for purposes of efficiency no more than 20 quoters will be awarded a BPA.
