- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is solicitation W81K02-18-Q-0028 under GFEB PR 0011157619 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97 The associated North American Industrial Classification System (NAICS) code for this procurement is 339115 (Surgical and Medical Instruments Manufacturing)) Size standard is 1000 This RFQ is being issued as Firm Fixed Price, 100% SET-ASIDE SMALL BUSINESS All responsible sources may submit a quotation which shall be considered by the agency. Endoscope shall have the ability to both open and check integrity of trabeculectomy sites. The probe shall be used in photo-coagulation procedures, by itself without requiring the use of further apparatus being introduced into the eye or incisions to be made. The device shall have the local service and representation with technical assistance 24/7. EVALUATION OF FACTORS: Award will be made based on Technical Acceptable Only. The evaluated price will be the total price of the quote, A. Must be FDA approved and TAA compliant. B. Must offer minimally invasive approach using Endoscopic Cyclophotocoagulation (ECP). C. Must combine endoscopic imaging and laser treatment for glaucoma and retinal disorder management. D. Must have an 810nm Diode Laser E. Must have a high resolution video camera. F. Must have option for 19G Ophthalmic Laser Endoscope both curved and straight. G. Must have a footswitch to operate the laser. Contractor must submit specification with the quote.
