- STATEMENT OF WORK General Statement: Contractor shall provide all material, labor, supervision, equipment and tools necessary to perform five year, annual, and semi-annual vertical transportation system inspections. Contractor s main office shall be within a two hundred mile radius to facilitate prompt service and to assist in emergent on site consultations. Subcontracting inspection or consultation services shall be prohibited and provided to the NORTHPORT VAMC at no additional charge. It is the responsibility of the inspector to determine if the equipment is in safe operating condition, that it has not been altered except in conformity to the applicable codes or regulations, and that each elevator (vertical transportation system) will perform in accordance with all applicable inspection requirements. In the performance of this contract the contractor shall take such safety precautions as NORTHPORT VAMC Fire/Safety and Engineering departments determine to be reasonably necessary to protect the lives and health of occupants of the building. Locations and Quantities: NORTHPORT VAMC is located at 79 Middleville Road, Northport NY 11768. The vertical transportation systems consist of (12) GEARED TRACTION devices and (10) HYDRAULIC devices for a total of (22) devices. Cost for inspection and inspection services provided shall be in the same dollar amount as the cost to provide inspection service for a similar vertical transportation system in the agreed upon area based on a firm fixed price. New elevators shall be inspected per applicable codes and regulations in force at the time of elevator construction design. For a complete listing of devices, building locations and types of devices refer to Attachment (A). Workmanship: All inspections, workmanship, and equipment shall be subject to re-inspections, examinations, and tests by government inspectors or their representative at any time during the course of work and at any place where work is being conducted. The NORTHPORT VAMC shall have the right to reject defective inspections or reports. Rejected inspections or reports shall be satisfactorily corrected and/or replaced without additional expense to the NORTHPORT VAMC. If the contractor fails or refuses to comply promptly, the Contracting Officer may issue an order, stopping all or part of the work, and hold the contractor in default. The Inspector is required to furnish tools and labor to perform the annual and semi-annual no load tests and Five Year load tests. The Five Year load test is to be performed in August 2021. The elevator maintenance and repair company shall provide elevator technicians and test weights as required for all inspections (five year load test). The elevator technician shall accompany the Inspector on all inspections. Inspections shall be conducted in accordance with NFPA 70, NFPA 70E, ASME A17.1 and A17.2 latest additions. Scheduling: Contractor shall perform annual inspections in the month of February and the semi- annual inspections in the month of August. Five year inspection due dates are indicated on Attachment (A) schedules shall not be altered without the consent of the Contracting Officers Representative (COR). The Inspector shall coordinate all inspections with the elevator maintenance and repair vendor and the COR. All inspections shall be coordinated at a minimum of thirty days prior to inspection dates. Inspections that may severely impact elevator service, or incapacitate a bank of elevators, shall be performed after normal working hours and approved in advance by the COR. All other inspections shall be accomplished during normal business hours (8:00AM to 4:30PM Monday through Friday except Holidays). Reports: Reports shall be formatted in Portable Document Format (PDF). Three copies and the original shall be submitted to the COR, Chief Engineering Service, Maintenance and Operations Manager, and Electric Shop Supervisor. Each report shall include a separate document for each device inspected. The report shall state the location of inspection, identifying device number (EIL#), type of device, all ASME deficiencies, reference section, number or letter designation of deficiencies, a heading and space to indicate scheduled date of repair, actual date of repair and date of inspection. All recommended upgrades shall be listed and identified as recommended upgrades on the same document. ASME Checklist for Inspection of Electric Elevators (A134C1 attachment (B), ASME Checklist for Inspection of Hydraulic Elevators (AO50C7 attachment (C) and 5 year load test attachment (D) ) shall be completed for each device inspected, dependent on type of device inspected. A certificate of inspection for each device listing the building, device number, type of duty, city, capacity, speed, date of inspection, elevator mechanic and/or elevator maintenance company, elevator inspector signature and inspectors qualified elevator inspector number shall be included on the certificate. This certificate shall be included in the report package. Report package shall be completed and delivered as stated above within thirty days of completion of the inspection. Past Performance Record: The Contractor shall provide information on Past Performance Record. This information is used in determining the level of confidence the Government has for a successful performance based upon the Contractor s actions under previously awarded and/or relevant contracts. The information provided in Volume III will be utilized to evaluate the Contractor s Past Performance. To be considered part of the Past Performance Record, a Contractor must demonstrate execution of three (3) or more relevant contracts in the last five (5) years. If a Contractor or its major subcontractor determines that they have no relevant past performance data, they shall provide the following statement in lieu of the information provided below: (Insert vendor or major subcontractor s name) has determined that they possess no relevant past performance information in accordance with the RFQ. To be complaint with this factor, Contractors shall provide the following information for both the vendor and proposed major subcontractors: (1) A list of Previous Contract Data for the Contractor. This list shall contain all relevant three (3) PRIME contracts and relevant subcontracts that the vendor is currently performing or has completed during the last five (5) years. The list shall include: - Contractor name - Name of customer/agency - Contract/Purchase Order Number - Customer points of contact (provide name, telephone number, and e-mail addresses) - Period of Performance (dates) - Description of product/service provided (include sufficient detail to demonstrate similarities/dissimilarities to the requirements of this solicitation) (2) A list of Previous Contract Data for all major subcontractors containing the information listed in (1) above as it relates to relevant Prime and Subcontracts that the major subcontractor is currently performing or has completed during the last five (5) years. (3) The Government may utilize the Federal Past Performance Information Retrieval System (PPIRS), the Contractor Performance Assessment Reporting System (CPARS), responses from the Performance Risk Assessment Questionnaire, data submitted by the vendor, and any other information available to determine the quality and relevance of the Contractor s past performance. Technical Capabilities: The offeror should demonstrate conformation with the following technical qualification prior to award. Understanding of the Requirements: The offeror must submit a proposal that demonstrates its understanding of work described in the Statement of Work (SOW). Contractor must also demonstrate the capability, experience and qualification to perform five year, annual, and semi-annual, vertical transportation system inspections. Contractor Qualification Qualifications and previous experience with similar contracts of this size (including Government as well as private sector contracts); Contractor to provide background, experience, resources, reputation, years in business, references. Contractor must be able to respond to the facility to the facility within 24 hours when an emergency service call is required. Contractor shall provide company certifications indicating that it is certified and experienced at performing five year, annual, and semi-annual, vertical transportation system inspections for the Government. Contractor shall provide certifications for the elevator inspector and an action plan to take on the elevator five year, annual, and semi-annual, vertical transportation system inspections services for the Government owned elevators. Inspection contractor shall provide proof of Qualification of Elevator Inspectors (QE-1) certification, a copy of inspectors QEI credentials shall be current and have no lapsed periods for a minimum of five years. All inspectors or inspector s assistants shall be an ASME/NAESA qualified elevator inspector with at least five years experience as a certified inspector. The Contractor s action plan shall include how the contractor intends to carry out the performance of elevator inspection services in accordance with TJC Standards, ASME A17.1 Safety Code for Elevators and Escalators, latest edition, ASME A17.2 Inspectors Manual for Elevators and Escalators, latest edition, ASME A17.3 Safety Code for Existing Elevators and Escalators, latest edition, ANSI/NFPA 70 National Electric Code, latest edition, and Manufacturer s requirements and specifications on inspection, maintenance, and operation of each elevator model. Service and Support The ability of the Contractor to meet delivery requirements within the time frame specified and how the understanding of problems that may occur; and how the contractor plans to meet them. This includes the stability of the Contractor s workforce; does the Contractor have an adequate number of employees and/or sub-contractors? Inspector shall be made available within a two day period from notification for additional onsite consultation and to clarify questions that may arise as a result of inspections at the discretion of the COR at the NORTHPORT VAMC at no additional charge. Inspector shall be available for phone, fax or e-mail consultation during normal business hours. f. Compliance with Specifications Followed instructions and thoroughness of RFP response. Emergent on Site consultation/inspections: Contractor s main office shall be within a two hundred mile radius to facilitate prompt service, and to assist in emergent on site consultations. Subcontracting inspections or consultation services shall be prohibited, and provided to the NORTHPORT VAMC at no additional charge. Security: Security certification and accreditation requirements do not apply. Security Accreditation Package is not required. All VA policies shall be followed and the contractor shall not come into contact with VA sensitive information.
