Description
The purpose of Amendment A00002 is to make the following changes to the Statement of Work. Changes are highlighted in red and underlined. 1. General Contractor shall furnish all equipment, labor, materials, apparatus, tools, parts, transportation, supervision, permits, disposal and expertise necessary to provide Preventive Maintenance and Emergency Services to the Hill Rom Hospital Beds listed below in paragraph 2.2 Equipment List with the exception of beds listed as Advance and Clinitron. Services for Advance beds will be limited to Preventive Maintenance only and services are not provided on Clinitron beds. The services will be provided at Bedford VA Hospital, 200 Springs Road, Bedford, MA 01730 2.3 The Preventive Maintenance shall include inspection and repair as required but not be limited to the following list of tasks as applicable for each model of bed according to the OEM service manual.: 2.5 Bed replacement parts shall be covered by this contract for beds with the exceltion of Advance and Clinitron and parts shall be replaced with manufacturer approved parts only. Contractor shall include a list of all parts covered by Preventive Maintenance to the COR. 2.5 The contractor will perform preventive maintenance in accordance with the OEM service manual for each model. If needed, contractor will supply a copy of the preventive maintenance section of each manual. Completion of the preventive maintenance work order by contractor will be verification that the work was performed as indicated in the manual. shall provide a check off sheet with each of the bed being inspected and received preventive maintenance. The check off sheet shall be turned into the COR for approval after the work is complete with 5 business days. 2.8 Contractor will maintain electronic records of all repairs and preventive maintenance, examples of which will be made available upon award. At the conclusion of each re pair and scheduled maintenance visit, the Co n tractor s hall provide a written service report indicating the elate of service, the model, serial number and location of equipment serviced, the name of the service representative, and the services performed and parts re placed. The Contractor shall maintain a permanent record for all beds. An electronic copy of this report, listing beds by their location s and serial numbers shall be emailed to the COR. 2.9 Emergency Services: Contractor will provides all services during normal business hours, Monday through Friday. The contractor shall provide 24/7-days response to emergency hospital bed problems at the VA Medical Center. Response time shall be required within a 2 hour time-frame. 4.3 Performance Requirements Table Required Service Standard ! l Performance Requirement Method of Surveillance Factors to consider for Option Year Renewal Contractor shall respond within two-hours by phone and schedule the on site visit at that time. Once call-in is placed, contractor has two hours to respond by phone. Contractor is required to respond for compliance COR Complaint that Contractor did not respond within two hours by phone for the emergency call Yes Contractor shall have ample supply of typical parts to repair beds for models that have repair service covered. Contractor shall have enough of their inventory to repair beds Contractor shall have parts available to repair beds to prevent beds being out of serviice for more than 5 work days COR Complaint if beds are out of service for more than 5 work days due to lack of parts available on hand by the Contractor Yes Contractor shall maintain electronic records of service.
