- Price Quote Info Sheet Floor Plan CID RFQ Combined Synopsis and Solicitation: W912DY-18-T-0131 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, in conjunction with FAR 13.5 as applicable, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97. Small Business set-aside is being conducted under this solicitation, the associated NAICS code is 337214 and the small business size standard is 1000. Please see the attached solicitation documents for PN 14CAMP009B, Loose Furniture, Inpatient Behavioral Health, BACH, Ft. Campbell, KY. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The provision at 52.212-2, Evaluation-Commercial Items, is used, and the specific evaluation criteria to be included in paragraph (a) of that provision is included in the attached solicitation. Offerors should include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition and the additional FAR clauses cited in the clause that are marked are applicable to the acquisition. This solicitation is to provide and install various furniture, equipment, and ancillary items for the PN 14CAMP009B, Loose Furniture, Inpatient Behavioral Health, BACH, Ft. Campbell, KY. The solicitation issued as a request for quotation (RFQ) is W912DY-18-T-0131, and is attached to this announcement. A list of line item number(s) and items, quantities, and units of measure is included in Attachment A. Site Visit: No site visit is anticipated.
