- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is W912CN-18-Q-0031 and this solicitation is issued as request for quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-97, effective 24 January 2018 and Defense Far Supplement Publication Notice DPN 20180131, effective 31 January 2018. (iv) This is a small business set-aside and the associated NAICS code is 813110. The requirements of this contract will be performed at the following locations on the island of Oahu, HI: AMR Chapel (Aliamanu Family Housing), 1790 Bougainvillea Loop, Honolulu Main Post Chapel, 790 McCornack Rd., Schofield Barracks Soldiers Chapel, Bldg 590, Schofield Barracks (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. The addendum is included below. ADDENDUM TO 52.212-1: INSTRUCTIONS TO OFFERS-COMMERCIAL ITEMS QUOTE SUBMITTAL INSTRUCTIONS 1. The request for quotation (RFQ) and evaluation of quotes will be conducted in accordance with FAR Part 12, Acquisition of Commercial Items, and FAR Part 13.106. Each interested contractor shall complete and submit the following forms, clauses, and statements with their quote. Failure to do so may cause a quote to be considered unacceptable. a) A completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items b) Amendments or acknowledgements (if applicable), signed and dated c) Submittal of quote in accordance with FAR 52.212-1(b) d) Responsibility - For purposes of conducting responsibility assessments, the contractor shall provide a narrative addressing the following, IAW FAR 9.104-1: (i) Adequacy of or have the ability to obtain financial resources to perform the required services (ii) Ability to comply with delivery or performance schedules, taking into consideration existing commercial and governmental business commitments (iii) Company organization, experience, accounting and operational controls, technical skills, technical equipment, and facilities or the ability to obtain them (including, as appropriate, such elements as production control procedures, property control systems, quality assurance measures, and safety programs applicable to the services to be performed by the prospective contractor or subcontractor). (iv) Description of how company/firm maintains business ethics and integrity. 2. Submission of quotes will be received through upload to the Government Point of Entry (GPE) or Federal Business Opportunities (fbo.gov) website. Lewis via email at Sebrina.A.Lewis.civ@mail.mil. a) Solicitation and amendment - Issued solicitation packages will be posted at the FBO (fbo.gov) website. Once posted, interested contractors are responsible to monitor the website until the closing date for any issued amendments. b) It is the responsibility of the contractor to ensure quotations are received and acknowledged by the contracting specialist, Sebrina A. Lewis, email: Sebrina.A.Lewis.civ@mail.mil on or before the deadline as stated. 3. Interested contractors shall submit questions in writing regarding the solicitation to the contract specialist via email at Sebrina.A.Lewis.civ@mail.mil. Final questions must be submitted no later than five (5) calendar days after solicitation issue date. A summary of questions received and Government responses will be provided via amendment and posted on the FBO website. Questions received after the seven day window may not be answered. 4. Materials not directly related to the information requested shall not be included in the quote. Any information not requested will be disregarded in the evaluation process. Unnecessarily elaborate quotes are neither necessary nor desirable. (ix) The provision 52.212-2, Evaluation -- Commercial Items, will be used, and the specific evaluation criteria is included below. ADDENDUM TO 52.212-2: EVALUATION- COMMERCIAL ITEMS: A. Basis of Award. The request for quotations uses streamlined simplified acquisition procedures pursuant to FAR Subpart 12.6 and FAR Subpart 13. The Government intends to award a single firm fixed price requirements contract resulting from this solicitation to the responsible vendor whose quote conforms to the solicitation, meets the minimum requirements, and will be the most advantageous to the Government. The following factors shall be used to evaluate vendors: Factor 1 - Ordination Status Factor 2 - Core Competencies Factor 3 - Price Basis for award will be to the vendor providing the lowest quote determined to meet the minimum requirements. Responsibility determination will be conducted on the apparent successful vendor only. Past performance will not be formally evaluated, but will be reviewed during the responsibility determination pursuant to FAR Part 9. As set forth in FAR 52.212-1, the Government intends to evaluate offers and award a contract without discussions with vendors. Therefore, the Vendor's initial offer should contain the vendor's best terms from a price and technical standpoint. The Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. B. Evaluation Criteria. Vendors are required to submit the following factors upon submission of quote. Factor 1 - Contractor shall have a valid ordination to Roman Catholic Priesthood, as evidenced by confirmation through the diocese from which the contractor was ordained. Factor 2 - Demonstration of core competencies as evidenced in a resume. Minimum Requirement: a) Contractor/employee shall have a working knowledge of the United States military and the military family lifestyle. b) The contractor shall be capable of working on ecumenical projects and demonstrate ability for teamwork in joint planning for community chapel events. c) The contractor shall have the ability to resource the Catholic Worship Community within the norms of the Roman Catholic Church doctrines and teachings. Factor 3 - Price Price will not be scored or rated and should be provided on the quote. The Government expects competitive quotes to be submitted for this procurement and will use these competitive quotes to determine price reasonableness in accordance with FAR 13.106-3. The lowest price quote acceptable for award will be evaluated based on the total contract price. If competitive quotes are not received, the Government reserves the right to utilize other methods identified in the FAR to determine price reasonableness. The Government intends to evaluate quotes and award without discussions; therefore quotes should be the contractor's best pricing. Definitions: Completeness/Accuracy: The Vendor's quote is in compliance with instructions in the Request for Quote. Reasonableness: A price is reasonable if, in its nature and amount, it does not exceed that which would be incurred by a prudent person in the conduct of competitive business (x) Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. (xi) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition.
