- Amendment 1: Question and Answer 1. Q. In which city is the site that requires the medical surveillance? A. South Burlington, VT 05403 2. Q. Is there a preferred month or pre-set dates for this medical surveillance or is it flexible? A. Beginning of 1 May to NLT May 18, 2018 7 MAY is not available for physical exams 3. Q. Please confirm the total for exams, vitals, and vision screenings is the count of 80 mentioned at the beginning of the RFQ. A. 80 unless otherwise annotated (Also 80 is the maximum number) 4. Q. Only a dermatological exam by physician is mentioned in the RFQ. Is a physical exam needed as well or strictly dermatological? A. Dermatological exam, vital signs to include height, weight, blood pressure, pulse and respirations. 5. Q. How many personnel are expected to require the Job Functional Analysis or Kraus-Weber? A. Doctor to determine if required; based job duties (expect about 70% of total) 6. Q. Will you accept a Board Certified Occupation Physician to review all exam and data, but not be present for the physical exam on site? A. We want the Board Certified Occupation Physician to be on site. 7. Q. Audiograms and vision screenings and vital signs including ht/wt blood pressure pulse and respirations. A. Maximum 80? 8. Q. Pulmonary Function Tests and Respiratory FIT test requirements A. Maximum 18 personnel 9. Q. Individuals that need blood work completed A. Maximum of 5 (Allied Trades Personnel) 10. Q. The request is for dermatological examinations. A. This is for all participants; maximum of 80 individuals 11. Q. Would we receive the prior audiometry data in an electronic format or on paper? A. We will provide a paper copy. This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. Quotes are being requested under RFQ W912LN-18-Q-0002. The Vermont Army National Guard intends to award a single firm fixed price purchase order using Simplified Acquisition Procedures using Lowest Price Technically Acceptable from a responsible supplier/vendor that meets all the technical Performance Work Statement Conditions. The NAICS code is 621999 and the business size standard is $15 M. This requirement is 100% set-aside for small business. Refer to PWS, QASP, and Provisions and Clauses attached
