Description

I. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. All vendors shall be registered at www.sam.gov and www.vip.vetbiz.gov Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov ). II. This solicitation is issued as a request for quotation (RFQ); submit written quotes on RFQ number 36C24618Q0546. This solicitation is a Brand Name Only request for Sterrad 100NX System Upgrade, to be delivered to the Department of Veterans Affairs (VA) Medical Center at 1601 Brenner Avenue, Salisbury, North Carolina 28144, which meets the requirements within this solicitation. III. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97. IV. This solicitation is set-aside 100% for Service-Disabled, Veteran-Owned Small Business concerns. The North American Industry Classification System Code is 339113 and the size standard is 750 employees. V. This combined solicitation/synopsis is for procurement of the following commercial supplies: CLIN Quantity MPN # Description 0001 1 100NX ALLCLEAR ECO-SYSTEM STARTER KIT 0002 1 10033ACSW ALLCLEAR TECH SW STNX 0003 1 10104ACSW ALLCLEAR TECH SW ST100NX VI. Required delivery within 60 days of award. Deliver all products to the VA Medical Center, 1601 Brenner Avenue, Salisbury, North Carolina 28144. Delivery shall be F.O.B. Destination. VII. FAR 52.212-1, Instructions to Vendors -- Commercial Items (OCT 2015), applies to this acquisition. VIII. The Government intends to award a firm, fixed priced contract to the responsible vendor whose quote, conforming to the requirements in this request for quote, will be most advantageous to the Government. A Lowest Price, Technically Acceptable award will be made based on the evaluation of technical experts. Technical acceptability is 1) the provision of brand name product in the Schedule, and 2) no gray market offers; all warranty and liability coverage shall be intact. IX. The vendor must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (DEC 2016), with its quote. A vendor shall complete only paragraph (b) of this provision if the vendor has completed the annual representations and certifications electronically at http://sam.gov. X. FAR clause 52.212-4, Contract Terms and Conditions - Commercial Items (MAY 2015), applies to this acquisition. Additionally, the following FAR clauses apply to this acquisition: 52.203-17 Contractor Employee Whistleblower Rights & Requirements to Inform Employees of Whistleblower Rights (APR 2014); 52.204-4 Printed or Copied Double-Sided on Recycled Paper (May 2011); 52.232-40 Providing Accelerated Payments to Small Business Subcontractors (DEC 2013). XI. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (JAN 2017) applies to this acquisition. The following clauses under subparagraph (b) apply: 52.204-10; 52.219-28; 52.222-3; 52.222-19; 52.222-21; 52.222-26; 52.222-36; 52.222-50; 52.223-18; 52.225-3 Alternate I; 52.225-13; 52.232-34. XII. The following VAAR clauses are also applicable to this acquisition: 852.203-70 Commercial Advertising (JAN 2008); 852.203-71 Display of Dept. of Veterans Affairs Hotline Poster (DEC 1992); 852.211-70 Service Data Manual (NOV 1984); 852.232-72 Electronic Submission of Payment Requests (NOV 2012); 852.246-70 Guarantee (JAN 2008); 852.246-71 Inspection (JAN 2008). XIII. The following VAAR provisions apply to this acquisition: 852.211-72 Technical Industry Standards (JAN 2008); 852.233-70 Protest Content/Alternative Dispute Resolution (JAN 2008); 852.233-71 Alternate Protest Procedure (JAN 1998); 852.273-70 Late Offers (JAN 2003); and 852.273-2  Alternate Evaluation (JAN 2003). XIV. Quotes are required to be received by the Contracting Officer electronically via email no later than 8:00 A.M. EDT on March 27, 2018; all quotes shall be emailed to darla.hurlock@va.gov. NO FAXED OR MAILED OFFERS WILL BE ACCEPTED. All submissions become part of the procurement file.
