This is a Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Background: The Robley Rex VA Medical Center (RRVAMC), located at 800 Zorn Avenue, Louisville, KY 40206, has a requirement for off-site Optometry services. The facility(ies) must be located within a 50-mile radius of the RRVAMC, 800 Zorn Avenue, Louisville, KY 40206. Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. NAICS code: 621320 and sized standard is $7.5M. NOTE: Respondent claiming SDVOSB and VOSB status must be registered and CVE verified in VetBiz Registry https://www.va.gov/osdbu/. Project requirements: See the attached Performance Work Statement (PWS) Anticipated period of performance: (July 1, 2018 June 30, 2019, plus four (4) one-year options Other important considerations: Cite any other information that is necessary for potential respondents to understand the nature of the potential requirement or proposed acquisition.   Capability statement /information sought: Response to this announcement shall not exceed 10 pages and should include the following information: (1) A tailored capability statement indicating the firm s ability to provide a product and related services; (2) The respondents DUNS number, organization name, address, point of contact, and size and type of business (e.g., SDVOSB/VOSB, etc,) pursuant to the applicable NAICS code; (3) Associated contract vehicles (i.e. GSA/FSS; SEWP); (4) Information should be provided electronically in a Microsoft Word or Adobe PDF format. Please e-mail your response with the above information to pretial.caston@va.gov or fax to 615-849-3789 with the subject line "Sources Sought Notice # 36C24918Q9393 by 12 noon Central Time on April 4, 2018.
