REQUEST FOR INFORMATION: 36C26218Q9227 VA LONG BEACH THIS REQUEST FOR INFORMATION (RFI) IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201(e). This is NOT a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified Service-Disabled Veteran-Owned Small Businesses (SDVOSB) or Veteran-Owned Small Businesses (VOSB) relative to NACIS 334510 sources (size standard 1250 Employees). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this source sought synopsis, a solicitation announcement may be published in the FedBizOpps or GSA eBuy websites. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. The Department of Veterans Affairs, Network Contracting Office 22, is seeking sources to provide the services listed below: SERVICES SOUGHT: Perform services to repair Mederi MDRF1 Generator. Vendor shall provide all necessary equipment, labor, materials, parts, personal protective equipment, supervision, tools, transportation, incidentals and non-personal services required to complete Mederi MDRF1 Generator repairs at the VA Long Beach Healthcare System (VALBHCS). Vendor must be certified / authorized to perform such repairs by equipment manufacturer at the contracted location. Vendor will provide individual written service reports which describe services performed on the equipment (in sufficient detail to be acceptable to field inspectors of The Joint Commission). The following information must be provided in each service report: 1) date of service, 2) summary of action taken, 3) labor hours, 4) travel time, 5) equipment serviced (description, equipment ID #, serial number, etc.), and 6) purchase order number. Vendor shall be engaged full-time in the performance of work requested / stated and shall have access to complete service manuals and software diagnostic keys. Vendor will perform services Monday through Friday from 7:00AM to 7:00PM, excluding federal holidays unless requested or approved by Biomedical Engineering. If you are interested, and capable of providing the required services please provide the requested information as indicated below. Responses to this notice should include: company name, address, point of contact, e-mail and phone number, and size of business pursuant to the following questions: (1) Please indicate the size status and representations of your business, such as but not limited to: Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Small Business, HUBZone, Woman Owned Small Business (WOSB), Large Business, etc.)? (2) Is your company considered small under the NAICS code identified under this RFI? (3) Does your company have an FSS contract with GSA or the NAC or are you a contract holder with NASA SEWP or any other federal contract? If so, please provide the contract type and number. (4) If you are an FSS GSA/NAC or NASA SEWP contract holder or other federal contract holder, are the referenced items/solutions available on your schedule/contract? (5) Please provide general pricing for your services for market research purposes. (6) Please submit your capabilities with regards to the services required and any information that can assist the Government (VA) in making acquisition decisions. Please include current contracts, licenses, certifications, etc. Responses to this notice shall be submitted via email to cecil.nichols@va.gov Telephone responses shall not be accepted. Responses must be received no later than Thursday, 03/19/2018 11:00AM PST. If a solicitation is issued it shall be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this request for information. Prospective bidders must be authorized re-sellers in order to participate in the solicitation process. Responses to this notice are not a request to be added to a prospective bidders list or to receive a copy of the solicitation.
