- The combined synopsis solicitation 36C10E18Q0117A00001 is amended as follows: The following are answers to vendors questions. QUESTION: What was the award amount for the last contract? ANSWER: This is a new requirement thus there is no award amount from a previous contract. QUESTION: What was the contract number or PIID? ANSWER: This is a new requirement thus there is no contract number from a previous contract. QUESTION: For the Fixed Unit Price declarations (years 1 thru 4) is states the following... " Unit Price___________ ;Quantity: 12; Unit of Issue: Month; Extended Price ____________ "... We are not sure what the Extended Price blank line represents ? ANSWER: So let s say for example that your unit price is $500.00 monthly then, the extended price is $500.00 times 12 months =$6,000.00 extended price or total price for the base year. QUESTION: We understand the importance of the Past Performance criteria. In this regard, we have attached 3 Letters of Reference. Will these Reference Letters satisfy the Past Performance requirements? ANSWER: Please follow the solicitation instructions for submitting the past performance information. QUESTION: In the S.O.W., Para 4.1.2 it states that we are to provide "initial screening and up to three hours of counseling sessions." Does the V.A. expect the initial screening (typically a 30 to 60 minute process) to be included in the up to 3 hours of counseling sessions... or does the V.A. consider the Initial Screening an additional EAP session event, separate from the max 3 hours of counseling? ANSWER: The screening is separate and the length will depend so just use an average length based on your experience QUESTION: Again in Para 4.1.2, we are responsible for the direct employee and their family members regarding EAP services rendered. In our past work contracts and experiences, we have typically used a Factor of 3... that is 2 Family members for each direct Employee, for a total of 3, on Average. In your opinion, is that a good Factor to use, or do you have other figures/statistics that show a different Average Family size for the 1,350 Employee? ANSWER: The intent is to use the 5% utilization rate from the 1,350 number.
