Description

This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in Federal Acquisition Regulation (FAR 12.6) as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued.   Solicitation number 36C25618Q0309 is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-84. The overall purpose is to provide and install a turn key Misting/ Fogging Disinfection System to disinfect patient rooms and public areas for the Southeast Louisiana Veterans Health Care System (SLVHCS), 2400 Canal St, New Orleans, LA 70119. All work shall be completed in accordance with the Statement of Work (SOW) entitled, Misting/Fogging Disinfection System dated 02/15/2018. The Statement of Work within this solicitation is a non-negotiable document. Offers/quotes shall not include edits, revisions, additions, deletions, alterations, or track change comments to the Statement of Work. The associated North American Industry Classification (NAICS Code) for this requirement is 339112 Surgical and Medical Instrument Manufacturing. This solicitation is set aside 100 % for Veteran Owned Small Businesses (Total VOSB set aside). Offers shall only be accepted from eligible Veteran Owned Small Businesses (VOSB s) (including Service Disabled Veteran Owned Small Businesses (SDVOSB s)). An eligible VOSB is defined as a Veteran Owned Small Business (VOSB) who has a non-expired, active profile/registration in the Vendor Information Pages (i.e. VetBiz https://www.vip.vetbiz.gov/ ). For your offer to be accepted (and for you to be considered for a contract award resulting from this solicitation) you must be an eligible VOSB with the appropriate active credentials in VetBiz. Upon receipt of offers, the contracting officer (or authorized representative) will verify that the offeror appears in VetBiz (i.e. The offeror has a non-expired, active profile/registration in this database). The contracting officer (or authorized representative) will verify the offeror s profile/registration in VetBiz through utilization of the offeror s DUNS numbers as it appears in the System for Award Management (SAM) database. Offeror s not appearing in the Vendor Information Pages (i.e. VetBiz) shall be removed from competition and shall not be considered for a contract award resulting from this solicitation. Offerors shall provide business name, doing business as (DBA) name, mailing address, phone number, DUNS number, CAGE code, and proof of active SAM (System for Award Management) registration when submitting offers in response to this solicitation. When preparing offers in response to this solicitation offerors shall refer to FAR 52.212-1 Instructions to Offerors-Commercial Items which provides specific guidance and instructions for offer submission. Offerors shall also familiarize themselves with 52.212-2 Evaluation-Commercial Items which provides the criteria for how offers will be evaluated
