Description
This is a REQUEST FOR INFORMATION (RFI) only pertaining to brand name or equal supplies outlined below for the Indianapolis VA healthcare facility, in Indianapolis, Indiana. Information collected during this RFI may be used in a set aside. If a solicitation is issued, the Government will do so in accordance with Federal Acquisition Circular (FAC) 2005-83. The North American Industry Classification System (NAICS) number is 339113. The NAICS size is 750 employees. Any contractor that believes they are capable and desires to claim preference for small business status must be registered with the SBA at http://web.sba.gov/pro-net/ and meet the requirements of FAR 19.102. Any contractor that believes they are capable and desires to claim preference for veteran owned small business status must be registered with the VIP at https://www.vip.vetbiz.gov as an SDVOSB or VOSB. Contractors that deem themselves capable of meeting the requirement shall provide the below information to Royce Jones, Contracting Specialist, at royce.jones@va.gov no-later-than Thursday, March 08, 2018 at 4:30PM, EDT. Responses shall include: (1) Business Name and Address, (2) FSS Contract Number if applicable (3) Point of Contact Name, Phone Number and E-mail Address (4) DUNS and NAICS code (5) Business Size (6) Type of Business: service disabled veteran owned, veteran owned small business, 8a, HUBZone, woman-owned, etc. Contractor must be registered with https://www.sam.gov Supplies are to include only TAA compliant items. Items that are not TAA compliant must be clearly identified by the Contractor should the contractor choose to offer a quote if a RFQ is published. OsteoCool System Salient Characteristics The VA s requirement is for a RF ablation system that is internally water cooled and bipolar. The system must include one (1) RF generator, one (1) pump, two (2) connector hubs, and one (1) cart. The OsteoCool RF Ablation System is indicated for the palliative treatment of malignant metastatic lesions in a vertebral body. The OsteoCool Bone Access Kits are indicated for percutaneous access to bone6. The RF Ablation Probe uses a coaxial, bipolar technology that delivers RF energy to treat the pain generated by metastatic spinal tumors in a localized and controlled procedure. The OsteoCool provides a thermocouple placed on the distal tip of the device to monitor the temperature occurring in the area around the distal tip of the probe. A pump unit offers water cooled RF ablation. The system provides percutaneous access to bone and has the option to use two ablation probes simultaneously. The system supports simultaneous treatment of adjacent or multiple levels, simultaneous Bi-Pedicular ablation, and Uni-pedicular ablation. It also provides ablation therapy for reduction or eradication of 1 or more bone tumors (eg. metastasis) including adjacent soft tissue when involved by tumor extension, percutaneous, including imaging guidance when performed.
