COMBINED SYNOPSIS/SOLICIATION 36C24618Q0400 Zeiss HFA3 Model 860 equipment (I)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this action. Quotes are being requested, and a written solicitation will not be issued. (II)This solicitation is issued as a Request for Quote (RFQ). Submit written quotes on RFQ 36C24618Q0400 (III)The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-96. (IV) Any amendments issued to this combined/synopsis will ONLY be available on the FBO website (www.fbo.gov). (V) The Network Contracting Office 6-SAO East intends to award a firm-fixed price contract for the VAMC DURHAM NC 27705-3897. (VI) The North American Industrial Classification System (NAICS) code for this procurement is 339115 with a small business size standard of 1000 employees. This procurement is a set-aside for SDVOSB and all eligible offerors may submit quotes. All offerors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award of a federal contract. The record must be complete and not missing elements such as representations and certifications. All SDVOSB s must be listed and registered in Vetbiz.gov to be considered for award. (VII) Questions concerning this solicitation shall be addressed to Audrey Wooten Contract Specialist, and emailed to Audrey.Wooten@va.gov. All questions or inquires must be submitted no later than 11am Eastern Standard time on 3/7/2018. No phone call will be accepted. (VIII) The Government will award a contract resulting from this RFQ to the responsible offeror whose quote is the lowest price technically acceptable. Quotes are due by 1pm. Eastern Standard time on 3/9/2018 Quotes received after this date and time will be considered late in accordance with 52.212-1(f) and will not be evaluated. Quotes shall be submitted via email to Audrey.wooten@va.gov (IX) This combined solicitation/synopsis is for the purchase of the following commercial items: Price schedule ITEM INFORMATION EYE VISUAL FIELD 266002-1145 773-PRO CARL ZEISS HFA3 MODEL860 W/LIQUIDTRIAL LENS, RELEYE 2 EA _______________ KINETIC Meeting Characteristics Salient Characteristics for HFA3 Model 850 Visual Field Analyzer HFA3 MODEL 860 W/ LIQUID TRIAL LENS, RELEYE, KINETIC, GPA, HEAD TRK, GAZE TRK, PRINTER & TABLE Performance required; or Essential physical characteristics; Dimension: 46 L x 52 W x 58 H (cm) which is exactly equivalent to: 18.11 L x 20.47 W x 22.83 H (inch) Electrical Requirement: 100-120v- 50/60 Hz, 4.0A Internal Storage: 500 Gb Operating System: Windows 7-64 bit Fixation Control Video eye monitor Heijl-Krakau blind spot monitor Gaze Tracking Head Tracking Vertex Monitoring Visual Field Index : Summary measurement of the patient s visual field status, expressed as a percent of a normal age-adjusted visual field. Liquid Trial Lens : Using Liquid pressure, delivers each patient s refractive correction. Available correction range is -8 to +8 diopters. Spherical correction only. Liquid Trial Lens available in model 860 only. SmartTouch interface: Touch screen interface allows for a more efficient workflow RelEye: The RelEYE fixation monitoring system allows for systematic and pin point fixation inspection. The HFA 3 captures and keeps the image of the eye during the testing and just after, prior to the Result Review Screen. RelEYE cannot be viewed on the HFA 3 after terminating the test. Using the HFA 3, RelEYE is also available in FORUM Glaucoma Workplace (minimum version 2.0.3). Only FORUM can keep the RelEYE data stored for future usage for exporting exams. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: Brand Name Only: This requirement is for VAMC Durham Eye Clinic located in North Carolina. The need is for the service of patient getting Visual Field Testing. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: Brand Name only is being requested for this procurement for standardization. The equipment will be an extension of the equipment currently in use across the board for the Durham, Greenville locations is paramount to safety, same type of training across the board and continuity of care. HVF tests done on HFA machines need to be compared over time to understand the progression of Visual fields for patients and is critical in developing safe and effective treatment plans. Description of market research conducted and results or statement why it was not conducted: Brand Name Only is being requested for this procurement for standardization. Market research has shown there are several SDVOSB vendors able to provide items regarding this requirement. Purchase will be open market set aside for SDVOSB.
