- AMENDMENT ONE. Amendment one is issued to provide Questiions and Answers. Offerors must acknowledge receipt of the amendment by completing Items 8 and 15, and returning 1 copy of the amendment with proposal. Questions/Answers (Q-1) The short listing description says the position is part-time, but the detailed description states that the position is five 8 hour days per week. How many hours and days per week is the position? (A-1) The position is a part time position for up to 20 hours per week. The exact schedule will be coordinated once the contractor is in place and would be during the times of Mon-Fri 8:00-4:30. (Q-2) What is the contract length? The listing says one year. Is there flexibility on the length of the contract? (A-2) The contract length is for a 12 month base year + (2) 12 month Option years. (Q-3) Would White Earth provide a dental assistant to me during the hours that I am there? (A-3) There would be a dental assistant provided while the provider is onsite. (Q-4) If a patient needed general anesthesia to complete a treatment plan, how would that be arranged? Many pediatric patients require general anesthesia in a hospital setting to complete the treatment. (A-4) Nitrous oxide is available onsite and general anesthesia is available as a referral to one of our other local clinics. The contractor would only be providing services at White Earth but could refer as needed to the OR. This is a Combined Synopsis/Solicitation For Commercial Items/services prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation will not issued. This solicitation is being issued using Simplified Acquisition Procedures under the authority of FAR Subpart 13.5 Test Program for Certain Commercial Items. This solicitation 18-239-SOL-00036; is a Request for Quotes(RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-96 effective Nov. 06, 2017. The NAICS code is 621210, and the small business size standard is $7.50. This is a total small business set aside. The Department of Health and Human Services/Indian Health Service has a requirement for Part time Pediatric Dentist Services for the Bemidji Area Indian Health Service. Must be a licensed Pediatric Dentist. Performance location is at the White Earth Health Center, located in Ogema, MN. Please see attachment for additional information. POC: karen.mcdonald@ihs.gov. The Government intends to award of a firm fixed price, contract resulting form this solicitation. Period of performance: Base year + 2 option years. Be advised that all interested parties must be registered in the System for Award Management (SAM) database. BASIS FOR AWARD: The Government intends to make an award without discussion therefore, the initial proposal shall contain the offeror's best price and shall be determined to be the overall best value and most advantageous to the Government.
