- Please note this is NOT a request for quote or proposal. The Government is seeking information strictly for market research purposes only. The Government may or may not issue a solicitation document. There is no solicitation at this time. This request for information does not constitute a request for proposals; submission of any information in response to this market survey is purely voluntary; the government assumes no financial responsibility for any costs incurred. The Robley Rex VA Medical Center, 800 Zorn Avenue, Louisville, KY is issuing this sources sought synopsis as a means of conducting market research to identify parties having an interest in providing On-Call Neurology Stroke Coverage Services 365 days per year, 24 hours per day/seven days per week. This on-call coverage is in accordance with the Performance Work Statement (PWS) attached. The purpose of this sources sought notice is to gain knowledge of potential qualified sources and their size classification relative to NAICS Code 621111 (business size standard $11.0 million per annum). Responses: If you are interested and are capable of providing the required services please provide the requested contact information. The intended contract period will be for one base year and four, one-year option periods. Responses to this notice should include a company name, address, point of contact (Name, Phone Number and e-Mail Address), DUNS number, business size, Socioeconomic Status (Veteran Owned, SDVOSB, HUBZONE, 8(a), etc.), and a capability statement.
