- This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this is to gain knowledge of potential qualified sources and their size relative to NAICS 811219, Standard Size $20.5 Million. Responses will be used by the Government to make appropriate acquisition decisions. Responses are not considered responses to any potential solicitation announcement. Scope of Work The Michael E. Debakey Medical Center located at 2002 Holcombe Blvd., Houston, TX 77030, requires a Contractor to furnish all labor, materials, parts, equipment, tools to perform extensive repair on the Davinci SI Series Laparoscope 12MM 0 D equipment. The contractor shall certify that the biomedical equipment is functioning properly and safely and ready for use. Contractor must provide 1 year of documentation that their company has worked on the equipment specified above. The vendor must also provide a service and maintenance agreement which will include adjusting, repairs, and replacing any malfunctioning equipment. Response to this notice should include a capability statement, and three (3) customer references to which you have provided similar services. All interested firms shall provide the following information: company name, address, point of contact, phone number, email address, capability statement, and business size pursuant to the NAICS Code 811219, Standard Size $20.5 Million. Responses to this notice shall be e-mailed to Jacqueline.Simpson3@va.gov. Telephone responses will not be accepted. Responses must be received in writing no later than Tuesday, March 06, 2018 at 10am, CST. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Responses are not a request to be added to a prospective bidders list or to receive a copy of the solicitation. STATEMENT OF WORK SCOPE OF WORK INTRODUCTION Michael E. Debakey Medical Center is requesting for a contractor to perform extensive repair on the Davinci SI Series Laparoscope 12MM 0 D (S/N SF1043055) equipment. This equipment is located at the Michael E. Debakey Medical Center located at 2002 Holcombe Blvd., Houston, TX 77030. DESCRIPTION OF WORK The Contractor shall furnish all labor, materials, parts, equipment, tools to perform extensive repair on the Davinci SI Series Laparoscope 12MM 0 D equipment. Contractor shall certify that the biomedical equipment is functioning properly and safely and ready for use. Contractor must provide 1 year of documentation that their company has worked on the equipment specified above. Following the inspection, contractor must demonstrate to the Contracting Officer Representative (COR) or designee that the equipment runs properly according to manufacturer specifications. The contract price shall be firm-fixed, all-inclusive, and not limited to specified products/equipment, installation, and setup/configuration. Contractor will test the Davinci SI Series laparoscope 12MM 0 D (S/N SF1043055) equipment to ensure it functions properly after repair. Contractor will provide a maintenance record on the Davinci SI Series laparoscope 12MM 0 D (S/N SF1043055) equipment listed above. The Contractor will not act outside of the scope of this project without guidance from the COR. PRIVACY/SECURITY The contractor will not have electronic connectivity to the devices and will not have access to patient databases or receive screenshots with PHI present. Any electronic or magnetic data storage devices such as hard drives, etc. shall be turned into the ISO. Electronic or magnetic data storage devices are not to be removed from VA property.
