Description

The Department of Veterans Affairs, VA Iliana Healthcare System VISN 12,1900 East Main Street Danville Illinois 61832, under the authority of FAR 13.5 (41 U.S.C. 1901 and implemented for restricting competition on this procurement via FAR 13.106-1(b)(2) ), awarded a contract on a sole source basis with Creative Health Care Management Inc. for continuance of Relationship Based Care Training Services. NAICS 611430, with a size standard of $11.0 Million applies. Contractor shall furnish all labor, materials, and services, including transportation cost necessary to provide Relationship Based Care Training Services for the VA Iliana Health Care System, 1900 East Main Street, Danville, Illinois for the period of Date of award to February 28.2019 to include an additional Two (2) One (1) Year options to be exercised at the discretion of the government. Services are to be provided in strict accordance with all schedules, specifications, terms, conditions and provisions of this contract. The approved Justification and Approval memorandum is posted as part of this award notice.
