Description
Lebanon VA Medical Center Sources Sought Medication Storage Cabinets The Department of Veterans Affairs, Networking Contracting Office (NCO), on behalf of Lebanon VA Medical Center (LVAMC), has need for custom built, in-wall pass through medication storage cabinets; general requirements summarized below. The anticipated contract will be an Indefinite Delivery Indefinite Quantity Contract, base year, plus four option years. This is a SOURCES SOUGHT announcement only. The purpose of this notice is to conduct market research and obtain information from qualified businesses. This is not a request for bids, quotes, or proposals. The Government is seeking to identify all Service Disabled Veteran Owned Small Business and Veteran Owned Small Business concerns, as well as all other responsible Small Business concerns, regardless of Socio-Economic Category, and other responsible business concerns with the desire and the capability to perform the work as summarized below. While SDVOSB/VOSB contractors are preferred, all capable contractors are welcome to respond to this sources sought notice for market research purposes. All responsible sources desiring this work must submit: A capability statement including proven capacity to perform this work (limited to 2 pages single spaced, one sided) Evidence of past experience as the prime or principal subcontractor on work of similar scope and nature; narrative/graphic demonstrating the work that has been done, (limited to 5 pages single spaced one sided, no more less than 15% white space per page for graphics) Evidence of recent (no more 5 years past) Satisfactory Past Performance of past work performed similar in scope, may include CPARS reports, customer certificates, accolades (limited to 5 pages single spaced one sided, no more less than 15% white space per page for graphics) Interested parties asserting SDVOSB/VOSB status must evidence include proof of CVE certification; that the entity is a verified Vendor Information Page (VIP) verified businesses, registered on https://www.vip.vetbiz.gov/ (limited to 2 pages) Interested parties asserting any other socio-economic must submit evidence of certification, i.e., SAM self-certification, completed Representations and Certifications. (limited to 2 pages) All small business firms, regardless of socio-economic status, that respond shall include proof of small business status via their Representations and Certifications in accordance with (FAR 4.1102 Policy). (limited to 5 pages) This requirement will be solicited under NAICS code 337127 and will be evaluated based on Lowest Price Technically Acceptable. Title of Project: Medication Storage Cabinets General Scope Of Work: INDEFINITE DELIVERY/INDEFINITE QUANTITY (IDIQ) CONTRACT: The vendor shall provide all resources necessary to accomplish the manufacture/custom build of in-wall, pass through medical storage cabinets. The contractor is guaranteed a minimum order of sixteen (16), a maximum total of sixty (60), medication storage cabinets that are designed to fit the prepared cavity. PERFORMANCE PERIOD: 5 Year, Firm Fixed Price, In-definite Delivery In-definite Quantity (IDIQ) GENERAL REQUIREMENTS: Cabinet must be stainless steel to meet drug storage requirements Cabinet must fit within existing wall opening, see attached drawing Med Cabinet Rough In Finished cabinet dimension: 14.5 w x 28 h x 9 d Doors are to be stainless steel Cabinet will be pass through, opening both to passageway and patient room Passageway opening Handles mounted centered vertically along the opening edge Must have one 13.5 wide by 24 tall door. Vertical continuous right side hinge. Have a HID Proximity/PIV reader compatible with integrating with the Lebanon VAMC Siemens PIV access system and also an alphanumeric Keypad lock. Patient Room (inside opening) One handle on each door mounted centered vertically along the opening edge Must have four 13 wide by 6 tall right hand swing steel doors. Vertical continuous right side hinge Have a Proximity/PIV reader compatible with integrating with the Lebanon VAMC Siemens PIV access system and also an alphanumeric Keypad Lock. Cabinet will have 3 adjustable/removable shelves, minimum 1 adjustments Shelves are to be antimicrobial coated steel. Contractor shall provide delivery of medication storage cabinets within forty-five (45) days of receipt. Shipping: FOB destination actual site TBD on each delivery order Interested parties submitting a response (total page limit 25 pages) to this inquiry must include, in addition to the above submission must include: name and address of company; business size (large or small); identify applicable socioeconomic categories, possible Federal Supply Schedule (FSS) contract number, and any pertinent information which demonstrates firms ability to meet the above requirement. All responses shall be submitted no later than March 8, 2018 at 3:00PM EST. All questions shall be emailed to: elizabeth.paugh@va.gov, sent with a delivery and read receipt requested.
