Description

The Network 10 Contracting Office, PO Box 492, Ann Arbor, Michigan, 48106, is soliciting RFQ VA250-17-Q-1148, which has been posted to FBO on or about February 22, 2018 - and intends to award a fixed price with Economic Price Adjustment, IDIQ contract for Adult Day Health Care services for beneficiaries of the VA Northern Indiana Health Care System, 2121 Lake Avenue, Fort Wayne, IN 46805. NAICS Code 624120, size standard $11 million.   This solicitation shall be conducted in accordance with FAR 13.5 and VAAR 873.   The Contractor shall provide Adult Day Healthcare services for veterans and must be located in one of the following counties: Elkhart and/or St. Joseph Counties.   The period of performance is one, five (5) year ordering period from April 5, 2018 through April 4, 2023, in strict accordance with all schedules, specifications, terms, conditions and provisions of the contract solicitation.   Quotes are due no later than 3:00 PM (Eastern) on March 12, 2018.   ANY AND ALL QUESTIONS regarding this RFQ are to be Emailed to: Michelle C. Ford, NCO 10 Contract Specialist, Michelle.Ford2@va.gov.   No Phone calls or Facsimiles will be accepted. Adult Day Health Care Centers shall remain registered in the SAM database after the initial registration for 5 years. The Contractor is required to review and update on an annual basis from the date of initial registration or subsequent updates its information in the SAM database to ensure it is current, accurate and complete, must also provide copies of current Certificate of Insurance as required by VAAR 852.237-7; and copy of most current Medicaid Rate/Waiver Letter.
